Job Description The Lead Scheduler's primary function is to facilitate scheduling of home visits for the field staff each day and assisting with the coordination of patient care. Ensuring geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care. This position will lead the other schedulers and act as a point of contact for the scheduling team. Essential Job Functions: Work with intake to push patients to scheduling status as quickly as possible to allow for accurate staffing Schedule all Starts of Care/Evals and Urgent PRN needs Monitor the other schedulers for accuracy with plotting scheduling communication notes, scheduler progress throughout the day, monitor referral log for updates on patients and keep statuses updated Ensures geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care. Schedules all new referrals to the appropriate clinician based upon location of the patient and clinician availability. Job Description Speech-Language Pathologist Reports To: Clinical Director or Rehab Director The speech-language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function. 2. Identifies functional speech defects and establishes a plan of care to improve patient's function. 3. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment. 4. Recommends communication devices/aids as indicated. 5. Confers with referring physician (or another authorized licensed independent practitioner) and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care. 6. Job Description Registered Nurse (RN) Reports to: Clinical Manager/Clinical Director The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care: Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan of care. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload.
