Position Overview
Bridgeway is seeking a Case Manager/Wellness Specialist, a critical member of a multi-disciplinary core services team, to provide wellness assessment and direct services to people who have serious mental illness and who are enrolled in the PACT Program: Program of Assertive Community Treatment. The ideal candidate will enjoy spending 70-80% of their time working with people.
The Case Manager Wellness Specialist for a PACT team will assume the following responsibilities:
Participate with the licensed staff in the development, implementation, and monitoring and updating of the individualized rehabilitation plan/Individual Recovery Plan (IRP).
As a team member in the frequent monitoring and assessment of the mental health status of persons receiving services as well as related variables, including significant others, the home and the community in which the person served lives
REQUIRED QUALIFICATIONS:
Master's degree and one-year experience in the provision of MH services

Bachelor's degree in a behavioral health science from an accredited institution and two years' experience in the provision of MH services

POSITION OVERVIEW:
Certified Screener to work onsite at North Plainfield and/or Franklin Police Departments. The screener will be co-responding with officers on mental health calls coming through police dispatch and/or PESS crisis line. Mental Health Screener and Police officer will arrive on scene together to de-escalate crisis and link individual with least restrictive environment of care. (i.e.: community mental health services/resources, voluntary hospitalization or start the process for involuntary hospitalization.) Certified screener can also schedule individual follow ups with individual in crisis to ensure linkage and no further escalation of crisis. For Fully Licensed Staff follow up services can include brief therapy.
Education and Experience:
RN degree or Master's degree in health-related field (social work, psychology)
NJ Licensed Social Worker, NJ Licensed Associate Marriage and Family Therapist or NJ Licensed Associate Counselor required
New Jersey Screener Certification or the ability to obtain certification within 1 year of employment
Prior experience as a NJ certified screener preferred

Position Overview:
Bridgeway is seeking a Team Leader who wants to lead a dynamic, multi-disciplinary team consisting of mental health clinicians, psychiatrist, registered nurses, peer specialists and co-occurring (behavioral and substance use) counselors. The ideal candidate should love working in an environment that offers flexibility and a collegial atmosphere. As part of a multi-disciplinary, core services team, the team leader will assess treatment needs, complete biopsychosocial assessments, provide and other direct individual counseling clinical to help ameliorate symptoms for adults with psychiatric disabilities. This position is 70-80% community based. The candidate who fills this position will also have administrative oversight of the team.
The Team Leader - will assume the following responsibilities:
o As team clinical and administrative supervisor in the frequent monitoring and assessment of the mental health
status of people served as well as related variables, including significant others, the home and the community in
which the person served lives.
o In the development of comprehensive assessment, coordination of all other assessments, and an individualized
service plan. Ensures documentation is done on time and with quality to ensure adherence to all
DMHAS/Medicaid standards.
Provides:
o Clinical supervision, leadership and direction to the PACT team, emphasizing a peer clinical supervision
approach and using individual supervision as needed. Provides monthly supervision for direct reports and
seeks out monthly supervision from own supervisor.
o Monthly individual, side-by-side sessions accompanying an individual staff member to meet with persons
served, family/significant others, and/or other service providers in regularly scheduled or crisis meetings
o Monthly structured meetings with staff as a group to address issues that cannot be addressed during the
daily, clinical, or treatment planning meetings or during the side-by-side sessions, to develop performance
and training goals, and teach of specific interventions/skills

o Assessment of service recipients stated concerns, mental illness symptoms, and behavior in response to
medication and monitors for medication side-effects during the provision of observed self-administration and
during ongoing face-to-face contacts
o Medication education to both individuals and family/significant others with service recipient consent.
o Coordination of services with other community MH and non-MH providers, and other medical professionals
o Ensuring that cases requiring more in-depth analysis and discussion among PACT team members are
reviewed at clinical case review meetings, to be held at least once per month

POSITION OVERVIEW:
The Per Diem Crisis Intervention Specialist, a bachelor's level behavioral health professional, is responsible for assessing each crisis outreach situation attended and deciding if additional psychological, psychiatric, or clinical services are needed. If so, the Crisis Intervention Specialist will consult with the Masters-Level Supervisor via remote connection for supervision and direction. - THIS IS AN IN-PERSON AND IN-COMMUNITY POSTION
REQUIRED QUALIFICATIONS:
Bachelor's Degree from an accredited institution in a mental health related field (psychology, social work, counseling, etc.) required
two-year post BA/BS professional experience in the MH field. Experience in a crisis setting preferred.
Valid driver's license required

POSITION OVERVIEW:
Provides program support and facilitates communication among the PACT team members and people served by PACT. Often serves as the initial point of contact for persons served or other providers calling or visiting the office. As a member of the PACT team, performs general program support functions and clerical duties as assigned with the program.
REQUIRED QUALIFICATIONS:
High School Graduate or equivalent required. Associates Degree or Graduate of Administrative Assistant/Business School Program preferred
Speak, read and write English (also Spanish speaking preferred)

REQUIRED QUALIFICATIONS:
Education and Experience:
Master's degree and one-year experience in the provision of MH services or,
