Summary: The CertifiedNursingAssistant will provide quality support to individuals with developmental and psychiatric disabilities.
Schedule: Monday 3:00pm-11:00pm , Tuesday 3:00pm-11:00pm , Thursday 3:00pm-11:00pm and Saturday 9:00am-11:00pm (38 Hours)
Essential Duties and Responsibilities:
Provide support and assist individuals with all activities of daily living which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements.
Provide care to individuals such as checking vital signs, checking their weight, applying creams/ointments, etc.
Maintain the comfort, privacy, and dignity of everyone in the delivery of services to them.
Observe residents for changes in condition or behavior and promptly report these changes to the Program Director.
Provide services to individuals according to DDS, CARF, and all applicable regulatory standards.
Provide support and assist individuals with all medical, clinical, and financial needs by identifying needs and securing appropriate services as needed.
Transport individuals as needed to and from medical, recreational, and other activities outside the program.
Maintain daily and monthly progress notes according to DDS and all applicable regulatory standards.
Write annual assessments on individual goals as outlined in the Individual Service Plan (ISP) and participate in the ISP meeting and other relevant meetings for the individuals.
Assist individuals with accessing resources and recreational opportunities in the community.
Administer medications according to MAP regulations.
Maintain core training certifications including CPR/FA, OSHA, CPI, PABC (if applicable), and MAP.
Other duties as assigned.
Education/Experience:
Must be a high school graduate with a minimum of 1 year of experience working with people with developmental or psychiatric disabilities.
Successful completion of a Massachusetts-approved NursingAssistantCertification course or other education that meets state certification requirements.
Must hold current NursingAssistantCertification in Massachusetts and maintain active certified status.
A bachelor's degree may be substituted for a CNA based on experience.
Knowledge/Skills:
Excellent written and oral communication skills.
Excellent organizational and problem-solving skills.
Valid driver's license and one year of driving experience.
Physical Demands:
While performing the duties of this job, employees are frequently required to climb stairs.
Must be able to lift to 50 pounds.
The employee must be able to drive a vehicle.
Must have a normal range of eyesight with or without corrective lenses.
Must have a normal range of hearing with or without corrective equipment.

The Care Partner, CNA / HHA will:
Assist with activities of daily living such as grooming, oral hygiene, toileting, bathing/showering, eating, etc.
Assist with personal and household chores, such as tidying up room, errands, delivering mail and/or newspapers.
Transport residents within the community to meals, enrichment activities, and other programs as needed.
Ensure resident care plans are reviewed and followed consistently.
Observe and monitor residents for behavioral changes and/or adverse reactions to medications or treatment plans. Communicate any changes to clinical staff immediately, as well as document changes thoroughly.
Respond to emergency calls, resident/visitor's requests and correct environment hazards in a timely, appropriate and safe manner.

We are looking for compassionate CNAs/HHAs to join our team! As a CertifiedNursingAssistant/Home Health Aide, your main role will be to deliver care to our residents within a warm, comfortable, and home-like environment.
Available schedules:
PT 1st shift and 2nd shift: Monday through Friday
Per Diem: all shifts, some weekends required
CNA/HHA Duties & Responsibilities:
Assisting residents with activities of daily living, such as bathing, dressing, grooming, and toileting
Documenting care provided and reporting any changes in resident health or behavior to appropriate staff.
Engaging residents in meaningful activities and providing emotional support.
Utilizing customer service skills to ensure that residents receive exceptional and meaningful care.

Summary: The CertifiedNursingAssistant will provide quality support to individuals with developmental and psychiatric disabilities.
Schedule: Saturday - Sunday 9:00am-5:00pm (16 hours)
Essential Duties and Responsibilities:
Provide support and assist individuals with all activities of daily living which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements.
Provide care to individuals such as checking vital signs, checking their weight, applying creams/ointments, etc.
Maintain the comfort, privacy, and dignity of everyone in the delivery of services to them.
Observe residents for changes in condition or behavior and promptly report these changes to the Program Director.
Provide services to individuals according to DDS, CARF, and all applicable regulatory standards.
Provide support and assist individuals with all medical, clinical, and financial needs by identifying needs and securing appropriate services as needed.
Transport individuals as needed to and from medical, recreational, and other activities outside the program.
Maintain daily and monthly progress notes according to DDS and all applicable regulatory standards.
Write annual assessments on individual goals as outlined in the Individual Service Plan (ISP) and participate in the ISP meeting and other relevant meetings for the individuals.
Assist individuals with accessing resources and recreational opportunities in the community.
Administer medications according to MAP regulations.
Maintain core training certifications including CPR/FA, OSHA, CPI, PABC (if applicable), and MAP.
Other duties as assigned.
Education/Experience:
Must be a high school graduate with a minimum of 1 year of experience working with people with developmental or psychiatric disabilities.
Successful completion of a Massachusetts-approved NursingAssistantCertification course or other education that meets state certification requirements.
Must hold current NursingAssistantCertification in Massachusetts and maintain active certified status.
A bachelor's degree may be substituted for a CNA based on experience.
Knowledge/Skills:
Excellent written and oral communication skills.
Excellent organizational and problem-solving skills.
Valid driver's license and one year of driving experience.
Physical Demands:
While performing the duties of this job, employees are frequently required to climb stairs.
Must be able to lift to 50 pounds.
The employee must be able to drive a vehicle.
Must have a normal range of eyesight with or without corrective lenses.
Must have a normal range of hearing with or without corrective equipment.
