Connecting People with Possibilities Mission: Bridgewell inspires hope and empowers people experiencing life challenges to achieve their fullest potential.Summary: The Housing Case Manager is responsible for supporting individuals in obtaining and maintaining housing, maximizing benefits, working with external agencies, establishing community relationships, and accessing internal resources as needed. Schedule: Monday-Friday 9:00am-5:00pm (40 Hours) Essential Duties & Responsibilities: Develop and maintain positive relationships with local service agencies, providers, realtors, and community landlords. Collect tenant rents and follow through on late rent collections including distribution of notices. Submit rents to Bridgewell financial services department. Conduct apartment inspections quarterly and/or as situation may warrant. Generate correspondence/reports for supervisor and follow-up letters to tenants as needed. Mediate with Property Manager common in-house disputes between tenants. Report maintenance needs reported by the tenant to Bridgewell Facilities Department in a timely manner. Provide support to individuals with completing HUD housing applications/education applications and other needs associated with housing and continuing education documents. Document housing progress for each tenant using HUD HMIS system and tier client medical records. Conduct individual and group informational/educational sessions focused on ensuring housing stabilization, housing challenges and other issues. Work with individuals on maximizing benefits and income including entitlements, work opportunities, and insurance management. Maintain required documentation according to HUD and all applicable regulatory standards. Maintain applicable core training certifications. Other duties as assigned. Required Education/Experience: Preferred educational background: Associate's Degree, or working towards degree in related field, or High School diploma or GED with a minimum of 2 years of successful employment within a housing and/or benefits maximization focused program. Experience managing individuals with substance abuse and mental health conditions. Required Skills/Knowledge: Knowledge of housing services, challenges, and resources within demographic area Knowledge of Massachusetts entitlements, housing subsidies, vocational opportunities, and State insurance programs. Excellent organizational and problem solving skills Warmth, empathy, and ability to set limits while maintaining a positive and respectful attitude. Flexibility, good judgment, organizational skills, and reliable Proficient in using computer, word processing, spreadsheets, navigation of internet Bilingual/bicultural preferred especially in Spanish or Khmer. These part-time positions are in support of On Demand Events, Surges, Summer Camps, and Backfills supporting the MFLC program. The Assignment Ready Counselor (ARC) will provide coverage in short-term, surge and on demand situations resulting from the Military needs such as a post-deployment event or to cover an existing assignment for counselors who are absent. The Summer ARC counselor position covers assignments during the May 15th through September 15th time period, coinciding with the school summer break. With minimal guidance and oversight, provides the full breadth of Adult or Child and Youth Behavioral (CYB) counseling services to military service members and their families at military installations. These services may include non- medical counseling, training/health and wellness presentations, consultation with parents, personnel at child daycare centers and schools, and consultation to installation command regarding behavioral health issues affecting military personnel and their family members. ARC CYB counselors may be assigned to DoDEA schools and community schools and will need to have experience working in a school setting to qualify for ARC role in backing up a school position. CYB counselors may also be assigned to youth programs, summer camps, and on demand assignments. ARC adult counselors may be assigned to support MFLC services in a variety of military community and readiness centers, as well as on-demand and surge assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation. This position has the primary responsibility of providing the full breadth of Child and Youth Behavioral (CYB) counseling services to children of all ages of military service members and their families at military installations to include civilian expeditionary work force and their families (when activated). These services may include non-medical counseling, training/health and wellness presentations, consultation with parents, personnel at child development centers, and consultation to installation command regarding behavioral health issues affecting children and youth. CYB counselors will be assigned to child development centers, youth programs, summer camps, and on demand assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation. Provides non-medical, short-term, solution focused counseling directly to eligible children and youth of service members at assigned primary and secondary locations. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated. CYB counselors will also have a focus on supporting the staff and personnel of Child Development Centers (CDC), youth programs and summer camps (if available/assigned) as well as providing parent support, training, and guidance. Provides training and health and wellness presentations, participate in health fairs and other base/installation activities focused on children and youth. Enters counselor activity data daily through smart phone or web application to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member. Summary: The FFS Clinician supports Bridgewell's mission by providing individual, family, and/or group psychotherapy and crisis intervention services to individuals with psychiatric and/or developmental disabilities. Schedule: Flexible schedule based on caseload 16+ hours per week Essential Duties and Responsibilities: Complete diagnostic intakes, treatment plans, goals and objectives, transfer/discharge summaries, and progress notes for assigned caseload. Provide individual and/or family counseling with clients on assigned caseloads. Coordinate services and collaborate with internal and external providers. Participate in clinical team and/or weekly supervision meetings. Participate in agency Clinical Risk Team Meetings (CRTs) as appropriate/requested. Maintain client records according to regulatory standards. Maintain productivity standards as discussed with the supervisor. Will be audited regularly to ensure compliance. Complete Utilization Review documentation follow-up promptly. Facilitate referrals to other agencies/vendors when appropriate. Adhere to all agency policies and procedures. Other duties as assigned by supervisor. Required Education/Experience: Masters Degree in social work, psychology, education, or counseling. Massachusetts state license preferred. Experience working with people with mental illness and/or developmental disabilities. Required Skills/Knowledge: Must have excellent verbal and written communication skills. Must have excellent organizational skills. Must have the ability to focus on and assess the clinical needs of clients and design indicated treatment goals and objectives. Must be self-motivated and able to work both independently and in a team setting. Physical Demands: While performing the duties of this job, the employee is frequently required to climb stairs. The employee must be able to drive a vehicle. Must have a normal range of eyesight with or without corrective lenses. Must have a normal range of hearing with or without corrective equipment.
