Summary: The Patient Care Coordinator is responsible for all administrative support to Bridgewell Clinic clients.
Schedule: Monday- Friday 9:00am-5:00pm (40 hours per week)
Essential Duties & Responsibilities:
Greet and assist visitors and direct all phone calls to the appropriate personnel.
Schedule all intake and follow-up appointments in the Electronic Health Record (EHR) for all prescribers.
Schedule new intakes in the EHR as applicable for salaried clinicians.
Support Intake/Registration Coordinator with all new referrals by ensuring that a copy of the patient's insurance card(s) is scanned into the EHR.
Collect current information such as address, phone number, emergency contacts, insurance coverage, and other key data at the time of scheduled appointments, and ensure updates are entered as necessary in the EHR.
Support optimal functioning of the clinic by sorting and distributing incoming mail, scanning all client documents into the EHR, and ensuring that refills are processed daily. Lab results should be reviewed by the prescribers each day and should be prioritized for scanning.
Maintain updated releases of information, annual "Know Your Rights" forms, and patient and contact information in the EHR, and handled requests for information (subpoenas and client requests) as needed. This may include obtaining records from off-site facilities.
Maintain organized reception, conference room, kitchen, and waiting room areas and ordered office and cleaning supplies as needed. Report any facilities-related issues to the landlord and/or the main office as needed.
Collect co-pays as applicable and record them in the EHR.
Prepare documents for the monthly Utilization Review process.
Obtain, monitor, and renew PT-1s (Prescriptions for Transportation).
Rotate to provide coverage in Bridgewell's other Outpatient Behavioral Health Clinics as needed.
Process inactive/terminated clinic cases, and maintain an annual list of client deaths.
Other duties as assigned by the supervisor
Required Education/Experience:
High School diploma required.
Experience with medical records, confidentiality regulations, and medical office operations.
Formal certification or education in any of these areas is preferred.
Candidate should demonstrate the ability to act as a team player and communicate effectively with a multi-disciplinary team.

Summary: The Developmental Specialist 3 will provide support and assistance in Level 3 day habilitation programs. Staff will provide support to individuals who need daily intervention, supervision, and skills training in activities of daily living within a day program environment and in the community. Individuals may require moderate to considerable physical assistance or accommodation. Staff will receive specialized training to provide enhanced supervision or interventions designed to address multiple disabilities, including behavioral supports.
Schedule: Monday - Friday 8:00AM - 4:00PM (40 Hours)
Essential Duties and Responsibilities:
Provide services to individuals according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards as well as Bridgewell policies and procedures.
Plan and implement an individualized program, based on each individual's needs following their Day Habilitation Service Plan.
Supervise the implementation of the Day Habilitation Service Plan and maintain the individual records in eHana.
Administer and interpret assessments to determine each individual's skill level on an annual basis.
Teach skills to individuals and small groups for training in designated areas.
Develop adaptive equipment and jigs to ensure individuals attain goals.
Assist people in making decisions about their daily routine/activities.
Assure that people have access to communication with family, friends, and significant others.
Ensure participation in community events, including attendance at volunteer sites, libraries, YMCAs, and any particular place of interest to them.
Assist with the upkeep and maintenance of the program, follow the cleaning schedule, report any maintenance issues, etc.
Maintain appropriate certifications including FA/CPR and MAP.
Maintain additional mandated training and certifications as required by the program.
Other Duties as assigned.
Required Education/Experience:
Bachelor's degree in Human Services or related field required.
Minimum of two years of experience in developmental disabilities or training programs with people with significant behavioral needs.
Training and/or experience in areas such as autism and/or behavioral support.
Required Skills/Knowledge:
Knowledge and understanding of principles of normalization, behavior management, and human rights.
Excellent written and oral communication skills so that writing required documents is clear and articulate.
Excellent organizational and problem-solving skills.
Valid driver's license and one year of driving experience.
Computer literacy includes the ability to proficiently use Microsoft products and web-based applications supported by the agency.
Physical Demands:
While performing the duties of this job, the employee is frequently required to climb stairs.
Must be able to lift up to 50 pounds.
The employee must be able to drive a vehicle and regularly meet the driving record requirements of the agency.
Must have a normal range of eyesight with or without corrective lenses.
Must have a normal range of hearing with or without corrective equipment.

Our Activity Therapists plan, direct and implement activity therapy programs as a component of the patient's treatment plan.
This is a full-time role with Tuesday - Saturday shift.
ESSENTIAL FUNCTIONS:
* Complete activity assessment through patient observation and patient/family interview, obtaining information on the patient's needs, skills, interests and limitations for the development and implementation of the treatment plan.
* Plan, organize, direct and implement the activities within the treatment program to facility patient progress with treatment goals.
* Plan and implement an activities program including but not limited to therapeutic leisure skills and activities, leisure education and leisure awareness.
* Facilitate and lead activity sessions to improve patient mental and physical well-being.
* Instruct patients in activities and techniques, such are sports, dance, music, art or relaxation techniques designed to meet their specific needs.
* Develop treatment plans and implement activity interventions that meet patient needs and interests and helps the patient achieve his/her treatment goals.
* Engage patients in therapeutic activities, such as exercise, games and community outings.
* Help patients learn social skills needed to become or remain independent.
* Modify activities to suit the needs of specific groups.
* Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
OTHER FUNCTIONS:
* Perform other functions and tasks as assigned.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
* Bachelor's Degree in Therapeutic Recreation, Recreational Therapy or related field required.
* Master's degree preferred.
* Three or more years' experience in recreation in a healthcare setting with knowledge of patient population served by the facility required.
* Previous experience in recreation in a healthcare setting is preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
* Certified Therapeutic Recreation Specialist (CTRS) or currently in the process of obtaining national certification; OR certified in the area of specialty (Art, Dance, Music, etc.) OR clinical professional licensure (LCPC, LPC, etc.) with certification in expressive therapy area required.
* CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
* First aid may be required based on state or facility requirements.
Pay Range: $30.44 to $36.35/hr.
