Summary: The Classroom Aide/Driver 2 transports individuals safely to and from day service programs as their Driver. As an Aide, duties include developing, implementing, and supervising a training program designed to meet each individual's needs, case management of designated individuals, and participation in general duties. Schedule: Monday-Friday 8:00am-4:00pm (40 hours per week) Essential Duties and Responsibilities: Work with and train individuals on specific day service plan objectives. Responsible for data collection of goals of each individual. Responsible for daily documentation of medical data or target behaviors. Participate in all general programmatic duties (arrivals, breaks, lunches, departures). Assist with the upkeep and maintenance of the program, follow the cleaning schedule, and report any maintenance issues. Protect each individual's rights and maintain confidentiality of any information relative to people. Complete any administrative duties as assigned. Participate in all required meetings. Maintain all appropriate certifications including FA/CPR and PABC. Assist all individuals in entering and exiting the vehicle and ensure all individuals have properly secured seatbelts. Interact with patience and sensitivity in all contact with individuals, family members, staff, and others. Operate the vehicle at all times within all federal, state, and local laws. Have good knowledge of routes and locations of all houses, day facilities, and emergency locations. Maintain and carry emergency fact sheets, daily attendance, and trip route sheets. Complete a quick vehicle safety checklist before beginning your assigned route and report any issues to your supervisor immediately. Immediately notify the supervisor of any variance in route time exceeding 15 minutes. Maintain all required certifications Other duties as assigned. Required Education/Experience: A minimum of a High School Diploma or GED is required. Must be 20.5 years of age or older, with at least 3 years of driving experience. Must have a valid MA driver's license and a good driving record history with no more than 2 moving violations in the last three years. Required Skills/Knowledge: Previous experience working with individuals with disabilities is preferred. Ability to interact with patience and sensitivity in all contact with individuals, residential staff, parents, and facility staff. Summary: The Program Supervisor assists the Assistant Director of Independent Living with oversight of the daily operation of the Individual Supports program for individuals with disabilities. Qualifications EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: Bachelor's Degree in Therapeutic Recreation, Recreational Therapy or related field required. Master's degree preferred. Three or more years' experience in recreation in a healthcare setting with knowledge of patient population served by the facility required. Previous experience in recreation in a healthcare setting is preferred. LICENSES/DESIGNATIONS/CERTIFICATIONS: Certified Therapeutic Recreation Specialist (CTRS) or currently in the process of obtaining national certification; OR certified in the area of specialty (Art, Dance, Music, etc.) OR clinical professional licensure (LCPC, LPC, etc.) with certification in expressive therapy area required. Summary: The Asleep Overnight CNA 3 will provide support and assistance to individuals with developmental and psychiatric disabilities during the overnight hours as needed in Level 3 residential programs. Staff will provide support to individuals who may need daily intervention, supervision, and skills training in activities of daily living while managing within a home environment and in the community. Individuals may require moderate to considerable physical assistance or accommodations. Staff will receive specialized training to provide enhanced supervision or interventions designed to address multiple disabilities which may include behavioral supports or enhanced medical care. Schedule: Wednesday-Saturday 11:00PM-9:00AM ASLEEP OVERNIGHT (40 hours) Essential Duties and Responsibilities: Provide support and assist individuals with all activities of daily living during the morning routine which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements. Provide care during the overnight shift to individuals who may wake up or have needs requiring assistance including checking vital signs, checking their weight, applying creams/ointments, etc. Provide care to individuals such as maintaining the comfort, privacy, and dignity of each individual in the delivery of services to them. Maintain safety of all individuals while on shift including implementing emergency policies as needed. Provide services to individuals according to, DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards, and Bridgewell policies and procedures. Maintain daily and monthly progress notes, shift checklists, and other program documentation according to DDS, DMH, and all applicable regulatory standards. Administer medications according to MAP regulations. Maintain core training certifications including CPR/FA, OSHA, PABC, and MAP. Maintain additional mandated training and certifications as required by the program. Complete household cleaning duties as assigned. Other duties as assigned. Required Education/Experience: Must be a high school graduate with a minimum of 1 year of experience working with people with developmental or psychiatric disabilities. Successful completion of a Massachusetts-approved Nursing Assistant Certification course or other education that meets state certification requirements. Must hold current Nursing Assistant Certification in Massachusetts and maintain active certified status. A bachelor's degree may be substituted for a CNA based on experience. Required Skills/Knowledge: Excellent written and oral communication skills so that the writing of required documents is clear and articulate. Excellent organizational and problem-solving skills. Valid driver's license and one year of driving experience. Computer literacy includes the ability to use proficiently Microsoft products and web-based applications used by the agency. Physical Demands: While performing the duties of this job, the employee is frequently required to climb stairs. Must be able to lift to 50 pounds.
