A caregiver in this position works varying shifts, with 48 hours/six-week period, weekend and holiday requirements. Orientation is 8 weeks, Monday to Friday, 8a-5p. Orientation can be flexible to 3 days per week. A caregiver who excels in this role will: Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols. Implement and monitor patient care plans. Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient. Perform accurate and complete admission assessments and reassessments within a specified time frame. Assess patient for safety risks, abuse and neglect. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Assess and document patients' pain using appropriate pain scale and manage patients' pain then. Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team. Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. Escalate change in patient condition to patient's physician as appropriate.
