Conduct Nursing and/or Needs Assessment at the start of care
Conduct meet and greets for new field staff placements
Monitor and coach home nursing and home services field staff
Review nursing notes and coach field staff to improve quality
Review weekly care notes for quality of documentation and changes in patient condition requiring revisions to the Plan of Care.
Case management: contact families, coordinate care, field visits - home and facility, communicate changes in Plan of Care. Obtain the physician's signature approval as necessary.

Rotating on-call schedule
Administer prescribed medications and treatments per approved nursing techniques
Conduct field supervisory visits and perform competency reviews on field staff skills as needed
Perform field injections, medication set-ups, and blood draws as time permits
Observe patient, record significant conditions and reactions, and notify supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents
Take and record patients' vital signs to detect deviations from normal and assess the condition of the patient
Respond to life-saving situations based on nursing standards, policies, procedures, and protocol
