    The Licensed Practical Nurse is responsible for providing direct skilled client care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes. A - Holds himself/herself and others accountable for getting the job done and maximizing value to company, clients, one another. Essential Job Functions/Responsibilities: 1. Provide direct patient care as defined under the state nurse practice act using sound judgment and critical thinking. 2. Implement plan of care and treatment initiated by the registered nurse. 3. Provide accurate, detailed and timely documentation consistent with the plan of care and the nursing process. 4. Conduct ongoing assessments. 5. Participate in health/therapeutic counseling, teaching and emotional support to the client and family/caregivers. 6. Observe client condition and accurately reports to the registered nurse. 7. Assist the registered nurse with client discharge or transfer. 8. Assess and provide client and family/caregiver education and information pertinent to diagnosis. 9. Prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous and intramuscular medications. Observe, report and record adverse reactions to medications and/or treatment to the registered nurse. 10. Recognize the need for and institute emergency measures in a rapid and appropriate manner, when indicated. 11. Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (i.e., PT, O, ST, Dietician) to the Director of Nursing or registered nurse supervisor. 12. Perform wound care. 13. Perform all skills, treatments and procedures competently in accordance to the scope of practice as defined in the Nurse Practice Act in the state of clinical practice and adheres to the policies, procedures and protocols of the agency. 14. Follow principles of infection control and Universal Precautions. 15. Identify and participate in performance improvement activities. 16. Use equipment properly and in a safe manner. 17. Ensure client's rights are adhered to. These include but are not limited to: a. the right to privacy and dignity while care is being provided b. the right to accept or refuse care c. the right to confidentiality of their information Qualifications: Graduated from an accredited practical or vocational school of nursing. Unencumbered license as a Practical Nurse in the state that care is delivered. 1 year of nursing experience required; three years nursing experience preferred. Community, home health or medical/surgical experience preferred. Comply with the state nurse practice act. Comply with accepted professional standards of practice. High level of interpersonal skills. Certified in CPR. Strong clinical skills. Excellent customer service skills and ability to deal tactfully with clients, other team members and community. Treat clients, team members and the public with courtesy, respect and presents a positive public image. Empathy for the needs of the client. Works as a team member. Self-directing with the ability to work with little direct supervision. Licensed driver with automobile that is insured in accordance with the organizations requirements. Environment, Personal Protective Equipment or Physical Demands Environment: Contact with clients under a wide variety of circumstances. Care provided in client's living environment with varying situations or in an institution such as a hospital. May be required to respond to emergency situations. Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues. Personal Protective Equipment: May include gloves, mask, eye/face protection, and disposable outer covering or lab coat. Physical Requirements: Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the ability of the client. Must be able to lift up to 50 pounds. Visual and hearing acuity required. Travel Requirements: Limited travel as needed for various staff and client care service needs. Up to 10% or as directed based on the needs of the business and attending job required training and conferences. Work Environment: Client's Environment: Unknown environments; bodily fluids, household and human germs; extreme cold or hot temperatures in or outside; light to heavy traffic; inclement weather; human body weights that vary based on client health and lifting as prescribed or needed per plan of care; 50+ lbs. Job Responsibilities Perform all skill, treatments, and procedures to the scope of practice as defined in the Nurse Practice Act - Ohio Revised Code (ORC) of clinical practice Implement treatment and plan of care, using sound judgement and critical thinking Maintain accurate, detailed, and timely documentation consistent with the plan of care and the nursing processes Document observed changes in patient's condition, situation, or care environment, and communicate to the Director of Nursing Adhere to the policies, procedures, and protocols of the agency of employment Adhere to HIPAA laws and ensure confidentiality and security of the client's medical information Job Requirements Unencumbered license as in the State of Ohio Minimum three (1) year of documented clinical experience. Private duty, home care assistance and/or direct care experience at an assisted living or skilled nursing facility, etc., is preferred Strong clinical skills. Self-directing with the ability to work with little direct supervision Certified CPR/BLS and negative TB skin test or chest x-ray Licensed driver with reliable and insured automobile (proof of insurance required) Ability to read, write, speak, and understand English and communicate effectively Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
