Develop a plan of care in partnership with the patient and/or family members. Create a realistic discharge plan incorporating the goals of the patient and/or family. Documentation: Document evaluations and treatment plans according to departmental guidelines. Record each treatment session following departmental protocols. Complete and submit family education, discharge, and other required documentation within facility and program guidelines. Comply with all Wellstar Health System policies, standards of work, and the code of conduct. Required Minimum Education: Master's or Bachelor's degree in Occupational Therapy. Required Minimum License(s) and Certification(s): Occupational Therapist licensure required. Basic Life Support (BLS) certification required.
