The RN Case Manager is responsible for overseeing the clinical care of Visiting Angels clients, ensuring that individualized care plans are developed, implemented, and maintained to the highest standard. This role serves as the clinical liaison between clients, families, caregivers, and office staff to ensure safe, effective, and compassionate home care. Key Responsibilities
Clinical Assessment & Care Planning
Conduct initial nursing assessments for new clients, evaluating medical history, functional status, and care needs.

Qualifications
Current Registered Nurse (RN) license in the state of Ohio (in good standing).

Strong assessment, critical thinking, and problem-solving skills.
Excellent communication and interpersonal abilities with clients, families, caregivers, and colleagues.

Initial Client Home Evaluations
RNs visit clients in their homes and provide initial evaluations of the physical, psychosocial, and environmental needs of clients and their families. These nurses help develop appropriate patient-centered care plans so that clients receive the best possible home care with respect to conditions or injuries. Nurses counsel clients and their families regarding self-care, chronic condition management, and evaluate clients responses to care provided.
Hospital Relations
Nurses act as the clinical care coordinator for assigned individuals and their families, collaborating with clients other healthcare professionals as needed. Registered nurses may also initiate referrals as necessary. RNs are a very important part of a ComForCare franchisees growth, as they form the cornerstone of the new client processing and outreach systems.
Working with Caregivers
After doing initial client home evaluations, RNs may help select an appropriate caregiver whose personality and skills are compatible with the needs and persona of the client. RNs will review care plans with caregivers to ensure understanding and help clients to receive the best care possible. RNs may also help to train and instruct caregivers on critical skills that may be necessary when assisting with condition management.
A Unique and Interesting Position
RNs that work with ComForCare offices are ideally those that love working with people and enjoy a regular change of scenery as opposed to an institutional-style setting. RNs generally do not have to participate in many front-line nursing activities, acting mainly as a position of authority and performing administrative-style duties.
RNs must be highly adaptable, able to work a flexible schedule and evaluate situations quickly and concisely to be a good fit with a ComForCare franchised business. Live the Mission The RN Infection Preventionist evaluates the quality of resident care and outcomes as they relate to Healthcare Acquired Infections (HAI) and Community Acquired Infections (CAI) in accordance with all applicable laws, regulations, and Life Care standards. Collects, prepares, and analyzes HAI data. Presents infection data and makes recommendations for actions. Monitors associate compliance with infection control standards through use of barriers and infection prevention measures. Prepares and presents education for the staff, residents, and families. Serves as a resource to all departments and personnel.
Education, Experience, and Licensure Requirements
* Nursing diploma (associate's or bachelor's degree in nursing)
* Licensed/registered in applicable State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.
* CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
* Specialized IPC training beyond initial professional training or education prior to assuming the role. If a facility's resident population changes, the IP should re-evaluate their knowledge and skills and may need to obtain additional training for the change in the facility's scope of care.
* Prior experience with geriatrics preferred
* Certifications or training per State requirements (e.g., Certified TB Nurse)
Specific Job Requirements
* Serves as designated associate responsible for monitoring Public Health Advisories from local, state, and federal agencies including the CDC
* Serves as the on-site IPC for COVID-19 prevention and response activities, in accordance with current CDC recommendations
* Works onsite at the physical facility. May not be an off-site consultant or perform the IP work at a separate location such as a corporate office or affiliated short term acute care facility
* Holds responsibility for the development and accountability of the Antibiotic Stewardship Program
* Maintains a current knowledge of infection control trends, methodologies, and employee health practices
* Performs work within the scope of his/her license
* Make independent decisions when circumstances warrant such action
* Knowledgeable of Infection Prevention and Control practices and procedures as well as the laws, regulations, and guidelines governing nursing functions in the post-acute care facility
* Implement and interpret the programs, goals, objectives, policies, and procedures of the Infection Prevention and Control Program
* Perform proficiently in all applicable competency areas
* Maintains professional working relationships with all associates, vendors, etc.
* Maintains confidentiality of all proprietary and/or confidential information
* Understand and follow company policies including harassment and compliance procedures

How we prepare our RNs for success:
* Paid training from day 1 in the office, in the home, and online
* Practice with award-winning adult and pediatric Simulation labs
* 24/7 on call clinical support
* Electronic charting using Statewise
We care for clients of all ages, diagnoses, and acuity levels. We offer a multitude of paid training to our nurses to feel comfortable and competent prior to working their first shift.
Available RN Shifts:
* Dayshift, evening or overnight
* 8's, 10's, or 12's (full shift work)
* PRN, Part-Time, or Full-Time
* No weekend or minimum requirements

Requirements:
* Current valid nursing license in the U.S.
