A typical day may include performing new client assessments, overseeing the therapies of existing clients, care coordination with skilled service organizations and our team of Home care Professionals. The Physical Therapist will work primarily in the field and rarely spend time in the main office. Travel in Washington County required. The Physical Therapist must be able to evaluate the patients condition and develop an individualized plan tailored to each client based on the physicians orders. Help clients enhance strength, range of motion, flexibility, and minimize pain or discomfort. Work as much or as little as you want, with our highly flexible schedule. This is a per-diem position. Starting wage is $75-$125 depending on type of visit. Benefits: Flexible Scheduling in Washington County and St Croix County Weekly pay with direct deposit 401K with Match PTO Create your own schedule Variety of assignments, procedures, and treatments One to one patient care Joint Commission accredited Responsibilities: Work cooperatively with physicians to develop care and treatment plan that is appropriate for clients home environment, especially as it relates to fall risk, interventions, and home safety Assess for muscle strength, mobility, gait, range of motion and pain and select appropriate treatment and assistive devices, instructing family and caregivers on use of devices Report clients response to treatment or changes in condition to the Director of Nursing and physician Initiate physical therapy program and instructs other personnel, including family members and caregivers, in specific phases of physical therapy with which they may Assist with the client care Prepare clinical and progress summaries, as well as participate in discharge planning. A copy of the summaries will be submitted to the Director of Nursing Treat clients, families, and staff with courtesy and respect Requirements Graduated from an accredited physical therapy program by the American Physical Therapy Association Valid licensure in state where care is given Comply with all state mandated Physical Therapy Practice Act and other legal mandates Adhere to HIPAA and maintain client confidentiality Requirements: Graduated with a masters or higher degree in physical therapy from an accredited physical therapy program Passed a National Physical Therapy Examination (NPTE) and/or state examination required by the state Wisconsin PT license required Two (2) years of appropriate experience as a physical therapist. Community/home health is preferred Adhere to HIPAA and maintain client confidentiality Certified CPR and negative TB skin test or chest x-ray Licensed driver with valid automobile insurance Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client.
