Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
Provides comprehensive evaluation and assessment of family and caregiver coping, and providing bereavement counseling and support.
Assists in financial assessments, which may include obtaining information to ensure insurance verification and payment.
Assists with education of potential patients and their family/caregivers to ensure informed consent, which may include obtaining signatures for consent for care and election of benefits for hospice care.

Qualifications include but are not limited to:
Master's degree (MSW) from a School of Social Work accredited by the Council on Social Work Education.
Minimum one (1) year of Medical Social Work experience required; Hospice and or Home Health Care preferred.
Electronic medical record experience preferred.
Ability to learn and assimilate the Sun Coast Computer Program/System.
Demonstrated ability to work with health team personnel. Bilingual (English/Spanish) a plus but not required.
Must have a valid drivers license and a willingness to use personal vehicle in the course of employment.
Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period).
Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)
