    Coordinates, plans, implements and organizes the ERSEA (eligibility, recruitment, selection, enrollment and assignment) functions in addition to planning, organizing, and implementing the Family and Community Partnership service area of the South Bronx Healthy Family Program. Responsibilities -Will identify, recruit and determine the eligibility of potential program participants based on the program ERSEA and EHS guidelines. -Will be responsible for ensuring that the program is at capacity and maintains a waiting list for applicants that are eligible for the EHS program upon the opening of space. -Will be responsible for rounding on the NICU, Women's health, labor/delivery, maternity floor, in addition to other community based partners to identify and ensure the screening and assessment of patients for program enrollment. -Will be responsible for maintaining the attendance of enrolled program participants. -Will be responsible for establishing and maintaining relationships with community based services, such as medical clinics, daycare facilities, preschool programs, Head start programs and other community based services to assist program participants with support services based on families need. -Will responsible for establishing and obtaining MOU from community partners. -Will input data into the computer and complete related reports in a timely fashion. -Will make home visits to complete applications and provide social service referrals to families, if needed. -Will assist with the organization and implementation of parenting committee. -Will ensure that goals of the Early Head Start program are met. -Will work with the Program Administration to develop specific plans for the implementation of program activities. -Will assist with special projects and perform other duties as delegated by the Administrative staff. -Audits and monitors ERSEA data and files for enrolled children. -Coordinates, plans, implements and organizes the ERSEA (eligibility, recruitment, selection, enrollment and assignment) functions of the South Bronx Healthy Family Program. -Makes presentations to community agencies regarding South Bronx Healthy Families.
