BronxWorks Positive Living provides services to improve the well-being of people living with HIV/AIDS, Sexually Transmitted Illnesses, and Hepatitis C. In addition, the program provides HIV & Hepatitis C rapid testing, health education groups to the community, and linkage to care and referrals for additional services. Our team is dedicated to helping everyone live their best life. RESPONSIBILITIES Provide direct services and prevention education clients living with HIV/AIDS (PLWH/A) and people who are at high risk for contracting HIV, sexually transmitted illnesses (STI's) and Hepatitis C, including people engaging in high-risk sexual behavior and intravenous drug users. Manage a caseload. Administer HIV and HCV Testing. Maintain familiarity with program resources available on- and off-site. Act as a program contact person for inquiries concerning clients. Facilitate evidence-based interventions, including, Healthy Conversations, Healthy Relationships and VOICES/VOCES. Provide education and increase awareness about PrEP and PEP and provide referrals to individuals in need of this service. Coordinate client services with other social services, mental health, substance use, medical and other providers through referral based program. Drive agency medical van to specified testing and outreach locations for the purpose of conducting testing and outreach responsibilities. Fill the vehicle with gas when needed. Notify supervisor of vehicle maintenance problems as they arise. Refer and link clients to medical provider. Complete intake, risk assessment, service plan development, referral, advocacy, and counseling. Conduct Outreach. Responsible for receiving dating and storing all food deliveries. Packing and distributing food packages to clients. Sorting food items, stocking shelves, checking for expiration dates and discarding expired items. Prepare and conduct educational workshops and presentations. Monitor and document client progress toward service plan goals. Provide weekly and monthly reports. Report to, and meet with supervisor monthly. Perform additional duties as assigned. QUALIFICATIONS Prefer at least one year of experience working with people living with HIV/AIDS. Prefer one year of experience doing HIV rapid testing and Hepatitis C rapid testing, although training will be provided. Minimum Qualifications High School Diploma. BA preferred. Bi-lingual English/Spanish preferred. Food Protection Certificate Preferred. Proficiency in modern business communications, including personal computers, electronic mail, voicemail, and facsimile and copier equipment. Valid NYS Driver's License is REQUIRED! The ideal candidate is passionate about teamwork, has some experience in office and administrative support, and has an interest in working with the populations we serve. Safe Options Support (SOS) teams address the needs of individuals experiencing homelessness on the street and in public transit locations. This is done by offering direct linkages to services that address their immediate needs, including transitional housing placement, connection to medical and mental health care, and access to benefits. Additionally, SOS provides support during the period of transition from street homelessness to stable housing. SOS Teams are comprised of licensed behavioral health clinicians, registered nurses, care managers, and peer specialists. This is an excellent opportunity to play a pivotal role in the Governor's newly launched, innovative Safe Options Support (SOS) program, which will provide comprehensive care to street homeless or subway dwelling individuals. Our full time Administrative Assistant is responsible for office operations including managing the reception desk, answering phones, greeting clients, data entry and records filing, receiving and sending mail, and supplies/equipment inventory and purchasing. Program Description: Goddard Riverside's Manhattanville (MNVL) Resource Center is a social service hub located in West Harlem in the MNVL NYCHA development, and which exclusively serves its residents. The MNVL Resource Center helps low-income individuals and families access and receive entitlements, benefits and resources. The MNVL Resource Center also offers an array of services including but not limited to, benefits screening, financial counseling, legal consultation and referrals to support services in the community and citywide. Position Summary: The Program Coordinator is responsible for integrating services, staff support, and assuming overall leadership of MNVL/Resource Center in the absence of the Assistant Program Director and Program Director. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational. Intro/Program Description: Lincoln Square Neighborhood Center (LSNC) is a multigenerational program of Goddard Riverside that serves the educational, recreational, and cultural needs of the Lincoln Square community, with a particular focus on residents of the Amsterdam Houses and Amsterdam Addition. LSNC Aging Services are funded by the New York City Department for the Aging (DFTA) and the New York State Office for the Aging (SOFA). More than 600 active members participate in the program, which provides daily nutritious hot lunches along with a wide range of wellness, educational, artistic, and social programming. Through comprehensive services-including case management, case assistance, and health promotion-LSNC Aging Services helps older adults (age 60+) lead vibrant, healthy, and meaningful lives. The Naturally Occurring Retirement Community (NORC) program specifically supports residents age 60+ of the Amsterdam Houses and Amsterdam Addition NYCHA developments. NORC services include social services, health promotion activities, and recreational, cultural, and social opportunities. Approximately 50% of participants are Spanish-speaking. The NORC operates in conjunction with the Older Adult Center, which is open to all New Yorkers ages 60 and older. Position Summary: Under the supervision of the LSNC Aging Services Program Director and the LSNC NORC Program Manager, the Case Assistant provides essential support and services to older adult clients. Responsibilities include completing new client registrations and annual assessments, updating client records, and reassessing client needs on an annual basis. The Case Assistant assists with applications and recertifications for benefits, makes referrals, and connects clients to critical services such as post-discharge homecare, legal assistance, and medical care. The role also involves client advocacy with public and private systems, collaboration with the on- site nurse, and providing telephone reassurance and friendly visiting. Additionally, the Case Assistant conducts home visits to ensure clients' safety and well-being. Schedule: Monday through Friday, 8:00 am to 4:00 pm. Weekends: Cooling Center Administration if required (occasionally, summer only, rotation schedule) Salary: $34,130.58 Qualifications Roles, Responsibilities, and Essential Duties: Under the supervision of the Program Director and LSNC NORC Manager, the Case Assistant provides direct client support and contributes to the effective operation of LSNC Aging Services. Responsibilities include: Client Services Provide advocacy, information, and referrals to connect clients with entitlement/benefits counseling, health and mental health care, food assistance, housing resources, caregiver referrals, and other services in line with program guidelines. Conduct new member registrations, annual assessments, re-registrations, and health indicator requirements. Perform home visits to homebound seniors to evaluate needs, ensure safety, link them to services, and provide ongoing social contact. Collaborate with the on-site nurse to coordinate medical care and assist with medication management. Support clients with scheduling and coordinating appointments, including providing reminders and follow-up. Deliver emergency outreach and assistance as needed. Provide friendly visiting and telephone reassurance to reduce isolation. Documentation and Compliance Maintain accurate, timely client records in both written and electronic formats. Enter data into client databases in accordance with agency and program standards. Team and Program Support Participate in staff meetings, supervision sessions, training programs, and case conferences. Contribute to program assessments, evaluations, and improvement plans. Assist with the daily oversight of senior center activities and respond appropriately to emergencies. Support operational needs, including lunch ticket distribution and front desk coverage, when required. Assist with outreach, engagement, and promotion of program services within the community.
