RESPONSIBILITIES:
Interactions with Clients, Community-based Service Providers

Documentation
Monitor and document client progress toward service plan goals; document all interactions with, or on behalf of, clients.
Complete program reports.
Review and sign off on service plans and psycho-social assessments as assigned.
Accurately and timely complete and compile 2010e (supportive housing applications).
Complete treatment plans as required.
Provide daily Bio-psycho-social assessment updates to Director of Social services and Residence Director.
Monitor and document client progress toward housing service plan goals/exit strategy.
Administrative
Complete biopsychosocial within 30 days of admission.
Manage a caseload for a diverse client population.
Provide group counseling as assigned by the supervisor.
Conduct regular home and field visits as required.
Conduct regular case conferences with clients and service providers as required.
Assist with special projects as assigned including representing agency at inter-agency events.
Act as program contact person for inquiries concerning clients.
Co-facilitate regular case conferences with clients and service providers as required.
Trainings/Supervision
Attend DHS CCC trainings/meetings as required
Attending individual and group supervision with the clinical supervisor as scheduled.
Report to, and meet with, supervisor on a minimum monthly basis.
Attend strategic planning supportive housing meetings as required.
Perform other duties as assigned by the manager.
QUALIFICATIONS
A master's degree in social work is required.
New York State LMSW certification required, or the ability to obtain a license within six (6) months of hire.
Proficiency in English required; proficiency in a second language preferred.
Strong oral and written communication, time management, and organizational skills are necessary.
Proficiency in Microsoft Office suite and other standard business technology is required.

Purpose of Position: The Stanley M. Isaacs Neighborhood Center (Isaacs Center) is seeking an experienced, dynamic, and highly focused individual to serve within the larger scope of formal and informal learning of the Education and Workforce Program. The LMSW - SocialWorker will work closely with participants ranging in age from 17+. They will carry their own case load of at least 40 individuals, with 20-25 meeting the qualification for bi-weekly counseling sessions. This person will work with their caseload with the multidisciplinary staff onsite to address barriers, formulate treatment plans and assist participants in meeting their goals. Weekly supervision will be provided by our onsite, LCSW - Clinical Director.

Cure Violence is a public health strategy to reduce and prevent shootings and killings. This program will focus on a defined area identified as a “hot spot” in Brownsville, Brooklyn for shooting violence within the 73

Precinct. The program works to prevent gun violence by: 1) mediating conflicts that may end in gun violence, 2) changing the thinking of individuals who are most at risk of being victims or perpetrators of violence, and 3) working closely with a broad coalition of local partners to promote a visible community-wide message that shooting is unacceptable behavior.

What The SocialWorker Does:
Invite/persuade referred clients to schedule initial appointment with Counselor.
Plan/Conduct family or support system meetings.

Coordinate services and provide feedback.

Program Description: Learning to Work (LTW) for Transfer Schools Program at Edward Reynolds West Side High School provides students over-aged and under-credited ages 16 -21 with services and opportunities to foster engagement through academic and personal development that lead to earning a high school diploma. Primary goals include getting students to graduate with realistic and meaningful post-secondary plans, gain knowledge and skills that will increase the employability and awareness of post-high school opportunities available to them. Some opportunities include higher education, trade/technical schools and community service programs. Using the Primary Person Model, each student has an LTW Staff Member as a support to minimize and/or remove barriers toward graduation including engaging students in college preparation, and meaningful career exploration including subsidized internships, academic and personal support services to increase self-direction and self-sufficiency.
Purpose of Position: The LTW Internship Specialist- Intern Specialist is the primary person support for a cohort of student interns who he/she knows well, counsels to promote goal-setting and supports towards successful completion of internship and the achievement of graduation with a meaningful postsecondary plan. The Intern Specialist will be instrumental in working with the Internship Coordinator to develop and manage the LTW internship program. This position will serve as liaison between students and worksites as well as connector to resources within and outside of the school.
Schedule: Part-time, Monday through Friday (25 hours a week)
Hourly Range: $17.12-$21.20

Case Management & Student Support Services:

Partner with LTW staff to prepare and present workshops and exploratory activities as needed for development of students' leadership, life skills, and self-efficacy
Support students to develop employability skills (Resume building, Cover letter writing, Mock Interviews, completion of internship application, job shadowing opportunities) through individual and group activities
Work with Internship Coordinator and worksites to ensure time and attendance are recorded properly and entered into our Paycom payroll management system
Record service/progress notes for students including but not limited to student interests and activities, SMART goals, attendance outreach, individual counseling, career and college advisement and exploration, service and volunteer activities, and other information as required by program. Assess for referrals to community based support services for students and family members
Assist Students not participating in subsidized internships provide with non-paid service learning and/or community service placements during the academic year, upon request and in collaboration with Internship Coordinator.
Additional Duties
Collaborate in the successful management of the internship program with Internship Coordinator, LTW Program Director, GRCC staff, NYCDOE Staff and the community.
Lead and/or champion school events to build community, pride and a welcoming environment.
Maintain and organize all required records and files according to organization and contractual requirements and standards
Collaborate with LTW Program Director with functions such as special events, performances, school community events
Attend and participate in-school / community-based meetings, administrative meetings, professional development trainings and other meetings as needed.
