Overview
Under the general supervision of the Business Manager and the Director of Practice Operations, provides direct nursing care for assigned patients at Pediatric Subspecialty, Bay Parkway Faculty Practice. Collaborates with providers, patients and their families, as well as other members of the health care team to formulate a plan of care and perform the duties of a registered nurse.
Responsibilities
A. ASSESSMENT
* Assessment, intervention, evaluation and health education are delivered within a knowledge framework of the growth and development stage appropriate to the patient.
* Initial observation and assessment of the patient's needs for immediate nursing action via telephone or walk-in triage.
* Assesses and intervenes appropriately to meet patient & families developmental, psychosocial, educational and home care needs.
* Plans and implements appropriate teaching about disease plan of treatment, side effects, diagnostic tests and psychosocial issues.
B. DIRECT PATIENT CARE
* Gives comprehensive patient care inclusive of but not limited to: immunizations, P.P.D.'s, medications, various injections, height and weight, head circumference, venipuncture, and repeat PKU's, allergy readings and other additional requirements requested by the provider.
* Collaborates with all members of the interdisciplinary health care team.

Responsibilities
Process prior authorizations for medications and schedule peer-to-peer consultations.
Complete school forms and disability/FMLA paperwork.
Draft letters for patients as required.
Triage emails and phone calls from patients.
Obtain lab reports as needed.
Follow up on Durable Medical Equipment (DME) orders.
Review incoming faxes from pharmacies regarding medication requests.
Conduct lung function testing (training will be provided).
Required Experience
Minimum of 2 years of RN experience required, preferably with pediatric experience. Experience in pulmonology and/or allergy is a plus, but not mandatory.

Assist in a variety of cardiovascular and cardiothoracic surgeries, including coronary artery bypass grafting (CABG), valve replacements, and aneurysm repairs.
Handle cases involving TAVRs, ECMO, Impella, Heartmate, lead extractions, and LVAD awareness.
Scrub and circulate within the operating room environment.
Required Experience / Certifications / Licensure
At least 4 years of CVOR and open heart experience is preferred.
Must have Scrub and Circulating experience.
Active New York RN License.
Valid ACLS and BLS certifications required; other certifications are welcome.

Carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, and identification of expected outcomes, planning, implementation and evaluation. Provides competent, safe and compassionate care based on established BronxCare Health System Patient Care and Nursing Standards. As a member of the interdisciplinary healthcare team, formulates the initial plan of care with the provider (physician or nurse practitioner) and collaborates with them to implement a comprehensive patient-centered plan of care to achieve expected outcomes; utilizes principles of assignment and delegation consistent with the NY State Nurse Practice Act.
Responsibilities
Provides professional nursing care to patients within unit. Diagnose and treat response to actual or potential health problems by means of nursing techniques and methods such as case finding, health teaching, health counseling and the provision of care supportive to and restorative of health and well-being. Maintain safe levels of care while adhering to a medical regimen. Collaborates with the physician and other health care providers to assure that a goal oriented care plan is consistent with medical regimen and hospital procedures. Performs other related duties as instructed.
Qualifications
BSN with three (3) years' experience preferred. Current NYS license to practice as a Registered Professional Nurse. BLS Required. Bilingual Preferred: English/Spanish.

Responsibilities
* Assessment, treatment and patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.
ASSESSMENT
* Identifies and documents initial observation and assessment of the patient's needs for immediate nursing action.
* Systematically and accurately assesses nursing needs of the patient in order to detect pertinent changes in the clinical picture.
* Identifies appropriate foci.
* Selects, plans, evaluates, and documents care based on this assessment. Documents patient's plan of care in focus charting format.
* Assesses, intervenes, and documents appropriately to meet patient's educational, psychosocial, and discharge planning needs.
* Initiates appropriate referrals to other departments, services, and/or outside agencies for patientcare.
DIRECT PATIENT CARE
* Gives comprehensive patient care.
* Initiates and follows through with patient and family teaching.
* Consults with unit Nursing Leadership regarding significant changes in patient's condition.
* Collaborates with all members of the health care team.
* Communicates patient care needs (foci) on shift-to-shift report.
* Utilizes patient care standards in clinical practice.
* Documents pertinent information regarding patient's response to treatment and progression toward goals in patient's chart.
* Writes comprehensive focus note which prioritizes patient care needs.
* Documents patient/family education in the Interdisciplinary Patient/Family Education form
* Seeks clarification of physician's orders that are unclear or inappropriate.
* Interprets clinical data and notifies the physician promptly if immediate changes in management are indicated.
* Administers intravenous medications in compliance with policies and procedures.
* Titrates medicated infusions as per physician's prescriptive, protocols, and standard.
SAFETY
* Checks to ensure that emergency equipment is on hand and in good working order.
* Recognizes and reports malfunction or failure of equipment used in the area to Biomedical Engineering; takes appropriate action to repair/replace such equipment (e.g., ventilators, cardiac monitors, defibrillators, suction apparatus, pumps, doppler, tank, NIPP, and pacemaker).
* Checks that all monitoring equipment and ventilator alarms are set according to standards.
INDIRECT PATIENT CARE ACTIVITIES
* Participates in patient care rounds by presenting findings on patients.
* Applies principles of infection control and safety.
* Assists in the orientation of new personnel.
* Provides clinical supervision of LPNs, Nursing Attendants, Patient Care Technicians, Unit Care Assistants and other ancillary staff.
* Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards; promotes adherence to Patient Bill of Rights.
* Indicates an Essential Job Function.
Qualifications
Graduation from accredited school of Nursing.
Current NYS licensure as Registered Nurse. BSN preferred.
Pay Range
USD $59,185.13 - USD $59,183.13 /Yr.
