Maimonides is seeking a Licensed SocialWorker to deliver brief, evidence‑based caregiver support services as part of the Caring for Caregivers (C4C) initiative for adults who are 65 years old and older and their identified care partners. This per‑diem role focuses on caregiver identification, outreach and consent, needs and quality tracking screening, goal‑aligned (“What Matters”) action planning, tailored resource linkage with teach‑back, EMR documentation, care‑team communication, and prompt time‑limited follow‑ups.
In this role, you will:
Identify and engage caregivers of adults who are 65 and older through admission/rounds flags and referrals; obtain consent and explain program scope.
Conduct brief structured screens using evidence-based tools (e.g., PHQ‑2/9, GAD‑2/7, brief caregiver‑burden check, health‑literacy and SDOH prompts).
Facilitate a concise “What Matters” conversation to elicit the caregiver's priority needs and preferences; co‑create an action plan focused on1-3 priority areas.
Provide tailored resource packets (education, benefits, respite, mental health, community‑based supports); use teach‑back to ensure understanding and feasibility.
Coordinate with the care team (e.g., primary team, case management, nursing, behavioral health) to align plans and communicate a concise summary note.
Track outputs and outcomes (encounters, screening completion, referrals provided, closed‑loop hand‑offs, caregiver‑reported stress/depressive symptoms when available).
We require:
MSW from an accredited School of Social Work; NYS licensure or permit as applicable.
Minimum 1 year in medical, geriatric, primary care, or community health social work experience with brief screening, goal‑setting, and resource navigation preferred.
Motivational interviewing or equivalent brief counseling; familiarity with evidence-based screening tools (eg, PHQ/GAD screening); knowledge of accessible and relevant caregiver resources (respite, benefits, mental health); EMR proficiency; strong communication and teamwork.
Diligent and effective follow-up; problem‑solving and sound judgment.
Hourly Pay: $45.81 - $64.77 (based on experience). Monday-Friday.

**Responsibilities**
Responsibilities include comprehensive psychosocial assessment, a full range of psychosocial interventions for patients/families, including transitional care; and ongoing efforts to engage patients with high utilization of emergency services after discharge in the community. Services will include counseling, including crisis intervention and assistance with concrete services (e.g., pharmacy, home care, transportation, entitlements, referral for community services), placement of patients in facilities and, close collaboration with socialworkers in outpatient settings. The socialworker in this position will meet with patients in-person in the emergency department setting at Mount Sinai Hospital and will continue to follow post- ED visit, enhancing linkages to primary and specialty care and community organizations.
The socialworker in this position will also have project management and development responsibilities specific to local and system efforts to respond to frequent utilizers of emergency medicine and inpatient care. The Mount Sinai Adolescent Health Center ("MSAHC" or the "Center") works to break down racial, social and economic barriers to health care and wellness for young people by providing vital services - high quality, comprehensive, integrated and confidential - for all who come to the Center or school based health facilities, at no cost to them. The MSAHC advances adolescent and young adult health as a national imperative by serving as a leading center of clinical care, specialized training, innovative research and advocacy.
The A28 SocialWorker will provide social work services to at risk adolescents and their families in an integrated medical and mental health primary care setting at MSAHC. She/he/they will provide short term individual, family, and group psychotherapy, intense case management, crisis intervention, and health education as part of an interdisciplinary medical and mental health service model. A28 SW will consult with medical providers who are working with high-risk patients with both medical and mental health needs and conduct clinical assessments on patients referred by medical providers. These include providers within primary care and child and adolescent psychiatry. **Responsibilities**
As part of an interdisciplinary team, the **socialworker will actively engage in the assessment and treatment planning of patients referred from medical providers in Article 28 setting while providing short-term therapy as needed. They will develop therapeutic alliance and deliver therapeutic interventions while consulting closely with medical providers and child and adolescent psychiatrist who are working with patients. The socialworker in this role will be expected to develop effective treatments plans and track treatment progress using validated instruments. As part of an interdisciplinary team, the socialworker will actively engage in the assessment and treatment planning of patients referred from medical providers in Article 28 setting while providing short-term therapy as needed. They will develop therapeutic alliance and deliver therapeutic interventions while consulting closely with medical providers and child and adolescent psychiatrist who are working with patients. The socialworker in this role will be expected to develop effective treatments plans and track treatment progress using validated instruments. * Perform intake assessments for patients new to the clinic.
* Provide direct treatment services to patients within the clinic utilizing appropriate modalities including individual therapy, group therapy, and crisis intervention.
* Provide presentations on pertinent clinical topics to trainees and fellow clinicians.
* Participate in interdisciplinary diagnostic and treatment planning conferences, including case presentations, and make recommendations regarding the needs of the patient from a social work perspective.
* Maintain established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.
Qualifications
* Masters in Social Work from an accredited school of social work.
* Current NYS licensure as a LMSW/LCSW.
* Minimum 1 year of experience in mental health field required.
* Experience working with children/adolescents in an outpatient psychiatric setting.
* Good interpersonal verbal/written communication skills.
* Bilingual is a plus.
Pay Range
USD $43.61 - USD $44.18 /Hr.

Under the direction of the Clinical Supervisor or designee is responsible for providing social work services in assisting in the resolution of social and emotional problems through appropriate psychosocial assessment and discharge planning for those referred patients on an inpatient medical, surgical, or pediatric unit, or ambulatory care setting.
Responsibilities
- Establishes and maintains positive relationships with patients, visitors and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of Bronx-Lebanon Hospital Center and its employees
-Carry an active caseload of patient/families as stipulated by departmental policies.
- Provide social work services to patients/families, including bio psychosocial/cultural assessment and treatment; crisis intervention, short or long term, as indicated and to include modalities as individuals, family, etc.
- Collaborate with inter-disciplinary team in providing patient care.
- Provide reports, record documentation and statistics as required.
- May serve as liaison within the department, hospital and to community agencies.
- May be delegated responsibility for in-service training teaching social work students, etc. Represent social work department in job related community meetings and conferences.
- Perform other activities as shall be deemed appropriate in assuring services to patients and the functioning of the Social Work Program.
Qualifications
- Sequence of study or experience in clinical practice in accordance with current New State standards for licensure and previous experience in the appropriate inpatient or outpatient setting as indicated.
