Position Summary:
-Off shift weekend/holiday Supervisors.
-Assumes shift responsibility for the overall assessment, planning, implementation and evaluation of patient care on the unit.
-Carries Hospital and Nursing Service mission, vision, values and the overall strategic planning, clinical management, patient care, budgeting, financial management and quality management on the unit.
-Promotes the health system service standards and creates a patient focused caring environment while addressing and meeting the individual needs of the patient.
-Fosters an environment of collaboration and respect with physician colleagues, nurses and other healthcare team members.
Principal Duties and Responsibilities:
Standard Compliance:
-Maintains current knowledge of and ensures compliance with all regulatory and professional standards of care which includes: CMS, IHI, CAP, etc.
-Utilizes creativity in developing quality/performance improvement programs, instructing staff and implementing new and improved standards.
-Utilizes results from “tracer” methodology tools to develop corrective action plans to ensure that quality of care is embedded into clinical practice on an ongoing basis.
Operational Performance:
-Develops, prepares and monitors the personnel, supply and equipment budgets in collaboration with the staff and support department directors.
-Develops long-term unit staffing schedules and ensures adequate daily coverage in accordance with established staffing needs.
-Develops measures and monitors unit operational performance metrics, agency and overtime costs.
Clinical Practice/Patient Care:
-Collaborates with the CNO to establish nursing care standards and evaluates staff compliance.
-Facilitates the effective provision of support services essential to patient care.
-Maintains a safe and caring environment for patients that is conductive to positive health teaching and maintenance; patient and family expectations of care to be delivered in a culturally sensitive manner and role models sensitivity to staff.
Management:
-Participates in the overall planning process for the service.
-Responsible for the overall planning process for the unit.
-Uses findings from patient satisfaction survey and with input from the staff, to establish unit based customer services agenda.
-Uses findings from staff satisfaction survey and with input from the staff, to establish a unit based recruitment and retention agenda.
-Develops an effective plan for delivery of nursing care within the assigned area of responsibility.
Human Resource Development/Management:
-Educates and evaluates staff.
-Educates staff in new and existing department procedures and systems (eg. Information systems, fiscal responsibility, etc.)
-Makes recommendations and provides opportunities for growth and development of personnel and arranges for appropriate education and training.
-Establishes unit metrics and goals to measure effectiveness of retention strategies.
Physical Demands:
-Involves standing, walking, sitting, talking, hearing and bending.
-May include some repetitive motions.

Under the general supervision of the Business Manager and the Director of Practice Operations, provides direct nursing care for assigned patients at Pediatric Subspecialty, Bay Parkway Faculty Practice. Collaborates with providers, patients and their families, as well as other members of the health care team to formulate a plan of care and perform the duties of a registered nurse.

A. ASSESSMENT
* Assessment, intervention, evaluation and health education are delivered within a knowledge framework of the growth and development stage appropriate to the patient.

* Assesses and intervenes appropriately to meet patient & families developmental, psychosocial, educational and home care needs.

B. DIRECT PATIENT CARE

* Initiates referrals to appropriate health professionals, home care services and/or community services as needed.
* Seeks clarification of authorized prescribed orders, protocols or standards that are unclear or inappropriate for age/population.
* Interprets clinical data and laboratory findings and notifies the physician promptly if immediate changes in management may be indicated. Documents findings and dispositions in the electronic medical record.
* Assesses and coordinates patient pain management.
C. EMERGENCY MEASURES
* Recognizes impending respiratory or cardiac failure and institutes appropriate measures of initiating CPR and calling 911 in an outpatient setting.
D. SAFETY

The Pediatric ICU (PICU) is seeking a skilled nurse to join their team. This unit is equipped with 11 beds and has telemetry capability, plus additional surge bed capacity managed by the same ICU-trained staff. The nurse will care for critically ill children in an environment that handles cases such as respiratory failure, trauma, surgical cases, sepsis, and neurosurgical complications.
Responsibilities
Provide high-quality care to pediatric patients in the ICU setting.
Monitor patients' critical conditions and maintain telemetry capabilities.
Float to Pediatric Emergency Room as needed, maintaining flexibility in duties.
Work in a nurse-to-patient ratio varying from 1:1, 1:2, or 1:3 based on patient acuity.
Required Experience / Certifications / Licensure
3 years of pediatric nursing experience preferred.
PALS and BLS certifications required.
PCAR certification preferred.
BSN preferred and CCRN certification is a plus.

4.* Applies antithrombic stockings under the directions of the RN.
5.* Assists with correctly positioning and turning patients in both bed and chair.
6.* Collects and sends specimens (e.g., urine, stool, sputum, blood).
7.* Performs foley catheter care and administers fleet enemas.
8.* Records intake (oral) and output (e.g., urinary, GI tubes and drains, vomitus and colostomy) accurately.
9.* Obtains and records height and weight of patient.
10.* Cares for patients in restraints as per Hospital Policy PROF #62.
11.* Assists RN with cardiac support and monitoring; applies cardiac monitor, admit/discharge from monitor and reports basic observations regarding skin color and moisture to the RN.
12.* Assists physician/nurse with treatments, diagnostic procedures (e.g., positions and holds patient).
13. Communicates effectively with Primary and/or Charge Nurse and other members of the
health care team regarding patient's needs, requests and/or changes in patient's condition.
14. Initiates age-appropriate skin care bundle for all patients at risk for pressure ulcers, incontinent patients and patients with stage I pressure ulcers.
15.* Assists RN with respiratory support: (not initial application), applies O2 face mask, nasal cannula, pulse oximetry, oral suctioning, chest PT and incentive spirometer. Prepares equipment for trach care.
16. Answers all call lights and patients requests in a timely fashion.
17.* Downloads EKG's and retrieves EKGs from data base.
18.* Transports patients to other rooms on current patient care unit.
B. PATIENT CARE-RELATED DUTIES
19. Performs hand off communication at bedside.
20.* Prepares equipment and patient for treatments or invasive procedures (e.g., sets up rooms, gathers equipment and trays, applies simple non-sterile dressings).
21.* Prepares pre- and post-operative beds; moves beds/furniture as necessary.
22.* Keeps patient unit and non-patient unit clean (e.g., wipes tables, cleans area, clean-up after procedures, cleans equipment between patients).
23.* Prepares area/unit with proper equipment for admission, transfer or discharge of patient.
24.* Answers telephones, prepares admission/discharge charts as required.
25.* Stocks and maintains supplies and equipment in area as directed (par level); reports need to restock to manager and/or designee.
26. Applies principles of infection control by following proper handwashing techniques, and isolation precautions.
27.* Makes frequent patient rounds to ensure patient safety and comfort.
28. Reports significant unit problems to charge nurse.
29.* Renders post mortem care (e.g., prepares body for morgue).
30.* Patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.
31.* Maintains established departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards. Promotes adherence to the Patient's Bill of Rights.
Qualifications
Education:
High School Diploma or High School Equivalency required.
Successful completion of Certified Nursing Assistant Program, or New York State Certified Nursing Assistant Program, or EMS Training.
Pass Patient Care Technician (PCT) Test (75% or greater).
Experience:
Minimum of 1 year of experience in direct patient care.
Skills:
Technical skills required for patient care.
Good oral and written communication skills.
Good interpersonal skills.
Speaks, reads and writes English to the extent required by the position.
