Responsibilities
Housekeepers clean floors, dust, remove trash, wet mop, wax and disinfect resident apartments, restrooms and public use spaces. Cleaning can include vacuuming, shampooing and deodorizing carpets, and cleaning surfaces throughout the community.
Respond to resident room emergencies, and log cleaning activities as required.
Housekeepers also interact with residents and guests in a friendly and courteous manner.

Position in this function supports corporate objectives by utilizing a demonstrated competency of Long-Term Services and Supports (LTSS) to provide Case Management. Participates in case conferences with the Supervisor.

**Primary Responsibilities:**
+ Intake/screening/referral, assessment/reassessment, development of support plans, on-going case management, monitoring of the Member's health and welfare, documentation of contacts and case management activities in the Department-prescribed system, resource development, and case closure
+ Respect confidentiality and maintain confidences as described in the UHG Employee Handbook and acknowledged through signature by all employees. Maintain confidentiality of patient information. The ability to maintain confidentiality is a critical and essential component of this position
+ Conduct monitoring contacts as prescribed by the Department. Contact the Member at least once within each quarterly period or more frequently as warranted by the Member's condition /or as determined by the rules of the LTSS Program in which the Member is enrolled
+ Conduct face-to-face contact with the Member's at least every six months or more frequently if warranted by the Member's condition or the rules of the LTSS Program in which the Member is enrolled. Update the current approved assessment tool and care or support plan in the appropriate documentation system(s) to reflect any changes in condition or services
+ Complete a new assessment during a face-to-face reassessment annually or more frequently if warranted by the Member's condition or if required by the rules of the LTSS Program in which the Member is enrolled. Complete additional assessments as needed/required
+ Monitor the delivery of services and supports identified within the Support Plan and the Prior Authorization Request (PAR)
+ Attend Department provided training for CMA agencies as well as agency provided in-service and staff development training
+ Responsible to conduct Work that is in accordance with the CMA Contract Agreement's requirements. This includes but is not limited to, providing Work that is accurate, timely, and complete
+ In the event, at any time throughout the case management process, the case manager suspects an individual to be a victim of mistreatment, abuse, neglect, exploitation or a harmful act, the case manager shall immediately refer the individual to the protective services section of the county department of social services of the individual's county of residence and/or the local law enforcement agency
+ May take on special assignments and projects on an as needed basis. Assist with system assessment and development as needed
+ This position requires travel. Requires independent, reliable, flexible, and on-demand, transportation at the incumbent's expense for travel between various locations and timely arrival and departure from various locations. Responsible for maintaining a secure environment for all residents and associates within the building and on the community grounds. Greets visitors, monitors the inside and outside of the building, and responds to emergencies.
* Greets and monitors visitors and residents coming and going from the building. Logs all residents and visitors in and out.
* Monitors the inside and outside of the building and the parking lot; reports any abnormal findings to the Executive Director or to the proper authorities as the situation dictates.
* Locks doors at designated time, according to community protocol. Checks the security of all doors regularly throughout the night.
* Maintains familiarity with and monitors all emergency and safety equipment including security cameras, phones, and life safety systems.
* Responds appropriately to emergencies by calling or contacting appropriate internal and external parties, according to established protocol. Assists with disaster management efforts and activities during evacuations (i.e. fires, hurricanes, etc.).
* Answers the phone promptly. Responds to questions, directs calls, or takes messages as appropriate.
* Escorts residents and visitors to the parking lot during evening hours, as requested.

The Advanced Practice Provider (APP) in Rheumatology is a key member of the clinical team, delivering comprehensive care to patients with rheumatologic conditions. This role combines clinical expertise, patient-centered care, and effective collaboration with a multidisciplinary team. As the Registered Nurse you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. You will also coordinate total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
Primary Responsibilities:
* Provide high-quality clinical services within scope of practice and infection control standards
* Coordinate care with other members of the patient/client's care team from admission to discharge
* Complete clinical nursing assessments per federal/state program requirements and payer needs
* Ensure patient/client eligibility and medical necessity for services as defined by payer and agency policy
* Develop and revise individualized plans of care/service plans with other community providers
* Ensure plan of care frequency and duration meets patient needs and initiate revisions with physician approval

Required Qualifications:
* Current and unrestricted RN licensure in the state of practice
* Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.
* Current CPR certification
* Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Preferred Qualifications:
* 1+ years of RN experience
* Able to work independently
* Good communication, writing, and organizational skills
$64,100 - $141,500 annual total cash target pay
$36.98 - $81.63 per visit point
$30.82 - $68.03 hourly rate
Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable.
