In this role, the provider develops, coordinates, implements, and delivers interdisciplinary services that integrate behavioral and biomedical science knowledge and techniques relevant to health and illness. These specialized services are applied to prevention, diagnosis, treatment, and rehabilitation efforts to support individuals on community supervision as they work toward successful community reintegration. Job Duties/Responsibilities Develops intervention goals and objectives designed to give education and resolve client's problems in functioning Participate in process of establishing, implementing, and evaluating case management system's goals and objectives. Staff cases with Parole Officers to assess and determine specific interventions techniques for clients and monitor progress. Respond to feedback on effectiveness and recommended treatment. Prepares data requiring social histories, admission/aftercare summaries, department documents and completes forms required by law and/or department policy. Serves on treatment teams and program development teams. Attends training meetings, intradepartmental meetings, and parole hearings. Establishes and maintains working relationship with other facilities, agencies, and support services in order to coordinate referrals and follow-up services. Knowledge of departmental regulations, policy, and procedures; governmental rules and regulations; counseling (group & individuals); client/patient care; crisis intervention; employee instruction. Ability to counsel on sensitive matters; proofread technical materials, recognize errors; and make corrections; recognize unusual or threatening conditions and take appropriate actions. Understand an abstract field of study; problem solve in situations with many variables; apply social work theories; principles & techniques. Organize and provide regular training for Parole Officers regarding serious mental illness, assessing functional abilities, practicing skill sets, community resources and other relevant topics.
