Identify changes in patient condition in conjunction with the interdisciplinary team and coordinate scheduling and completion of indicated assessments. Assist with Activities of Daily Living training for new hires and through ongoing continuing education to existing staff. Attend educational programs to maintain knowledge with current RAI/Prospective Payment System regulations.
