POSITION SUMMARY: Under limited supervision, the APP will deliver patient-centered primary care services, including appropriate assessment, diagnosing, treatment and management of patient's healthcare needs across the lifespan, with an emphasis on providing care to underserved populations focused on preventative healthcare, chronic disease management and patient education. APP will work as an integral part of HCCC integrative health care team by participating in regular meetings and quality improvement.
