As a Housing Liaison, your primary role is to engage individuals with disabilities in the community or persons in recovery and assist them in meeting their basic needs, including finding safe and affordable housing. You will actively assist clients in resolving housing crisis situations, serve as the internal expert on housing resources in your designated service area, and collaborate with local Continuums of Care to identify individuals experiencing unstable housing and establish a network for supportive and stable housing.

Key Responsibilities:
* Practice Housing First, harm reduction, and low-barrier service models when working with individuals or households.
* Attend and participate in Coordinated Entry/Case Conferencing meetings within the local Continuum of Care.
* Reach out to households currently not engaged in CPR or CSTAR services that are experiencing a housing crisis or homelessness.
* Refer and/or transport individuals to help establish them in services for needs including behavioral health, substance use, physical health, employment, etc.
* Build and strengthen relationships with local landlords, providing education on community programs and referrals for their units.
* Connect with referrals from Continuum of Care programs to assist individuals with housing stability.
* Utilize SOAR training to assist individuals in applying for benefits.
* Assist households in identifying when they are ready to move on to other permanent housing options.
* Participate in state-wide Housing Liaison training and calls.
* Complete timely data collection and entry as required by DMH.
* Become knowledgeable about all resources available within the community.
* Serve as a resource to case managers, clients, and agency administration regarding housing and local/state resources.
* Assist eligible households in obtaining identification documents required for housing or employment.
* Help with the moving on process/transferring to other services and meet with tenants to evaluate their readiness to move on to other permanent housing options.
* Reengage active or former participants who have become disconnected with services or who are at risk of returning to homelessness.
* Help households to identify and connect with natural resources to prevent or end homelessness.
* Understand HUD Quality Standards inspections.
Education and/or Experience Qualifications:
* A Bachelor's in Social Work, Psychology, Sociology, or other relevant case manager experience in housing; OR,
* Associate of Applied Science in Behavioral Health Support from an approved institution eligible to provide community support services; OR,
* Qualified to provide Community Support through specified educational background and experience as defined in community-support qualifications.

Key Responsibilities:
* Establish ongoing relationships with law enforcement, court, and jail personnel, including County Sheriffs, Municipal Police, Crisis Intervention Team (CIT) Officers, and more.
* Coordinate services with local systems of care for individuals with behavioral health needs, facilitating access to both inpatient and outpatient services.
* Follow up with referred individuals to monitor treatment and encourage attendance, addressing any barriers to care.
* Facilitate civil commitment procedures and provide education on guardianship processes.
* Collaborate with Access Crisis Intervention (ACI) to assess and coordinate services for individuals frequently in contact with law enforcement.
* Participate in local and state CIT meetings, maintaining regular contact with law enforcement and court personnel.
* Provide support and training to court and jail personnel regarding behavioral health services.
* Complete reporting requirements, entering all referral data through CareManager in a timely manner.
Education, Experience, and/or Credential Qualifications:
* Requires a master's degree or higher in psychology, social work, counseling, or a related behavioral health field and be an experienced, qualified mental health professional.
* Prefer LMSW, LCSW, PLPC, LPC.

* Successful completion of background checks including criminal record, driving record, abuse/neglect, and fingerprint checks.
* Current driver's license, acceptable driving record, and current auto insurance.

Location: Onsite at the Friends in Action Clubhouse. Our EWH (evening, weekend, holiday program) may be offsite at a park, in the community shopping/out to eat, at a parade, etc.
Who will this person interact with on a regular basis? All Clubhouse members daily who attend, we have about 35-50 members a day at the Clubhouse.
MUST HAVE skills for this position: self-starter, ability to lead a large group, ability to move from task to task, ability to guide/cheerlead a Clubhouse member to get that person started on a task and then move on to the next member, then back to the first member. This open position is for the Business Unit so would prefer computer skills including Excel, Publisher, etc.

Responsibilities
Management of Clubhouse Units- Organize and complete daily unit work with membership side by side/oversee assigned areas- Apply and model the Clubhouse Standards, as stated through the Clubhouse International - Creatively encourage members to participate in their rehabilitation through the WOD by making Clubhouse work urgent and meaningful - Plan and participate in the EWH program (Evening, Weekend, and Holiday activities) - Actively participate in the fundraising efforts of the Clubhouse - Complete frequent outreach of members who are not in attendance - Assist to meet the community needs of members - Provide crisis intervention services as needed

Participate in departmental, agency and interagency meetings as assigned.
Assist with continuous quality improvement efforts for this service.
Must be able to facilitate and manage crisis situations.
Must be able to help cover crisis access services, when needed.

Job Summary:

Key Responsibilities:
