Purpose of the Position
To provide transportation to meet the needs of Springvale Independent Living (SIL), Springvale Assisted Living (SAL), Long Term Care(LTC) and the Humboldt County community during daily business operations, weekday 8:30a.m. - 5:00p.m. This position will reasonably meet the transportation needs for individual scheduled appointments, group shopping, personal requested trips, and senior living group activities off campus. To help maintain a clean, neat, and safe environment with equipment in working order in accordance with the Federal, State, and local regulations that govern Humboldt County Memorial Hospital (HCMH).
Responsibilities
Follows a daily schedule of appointments made by consumer via the HCMH Therapy Registration team and carries it out to meet the resident's needs.
Completes errands on time for all areas of campus, as needed. Example: Lab deliveries or maintenance pickups in Fort Dodge
Assists SIL, SAL and LTC with shopping needs, as needed.
Effectively communicates with residents, co-workers, and Supervisors to ensure schedules are accurate for transporting consumers.
Clean and vacuum the interiors and ensure that all specialized equipment, such as wheelchair lifts, is in proper working order. Transportation driver will also wash and polish the outside of the bus to keep it clean and attractive. They may take the bus to a mechanic for routine maintenance or when the specialized wheelchair equipment doesn't function properly.
Able to collect and calculate transportation fares and vouchers from consumers.
Can safely detach the wheelchair security device, lower the patient out of the bus using the lift, and wheel the patient to his/her destination.
Completes in-service training and MedTrainer courses in a timely manner.
Responsible for keeping active Chauffeur's License (class D) license by following all DOT requirements.
Performs various ground keeping and maintenance tasks/repairs as requested.

Transportation Driver
Supervised by: Tanya List, Director of Therapy/Transportation
Purpose of the Position
To provide transportation to meet the needs of Springvale Independent Living (SIL), Springvale Assisted Living (SAL), Long Term Care(LTC) and the Humboldt County community during daily business operations, weekday 8:30a.m. - 5:00p.m. This position will reasonably meet the transportation needs for individual scheduled appointments, group shopping, personal requested trips, and senior living group activities off campus. To help maintain a clean, neat, and safe environment with equipment in working order in accordance with the Federal, State, and local regulations that govern Humboldt County Memorial Hospital (HCMH).
Responsibilities
Follows a daily schedule of appointments made by consumer via the HCMH Therapy Registration team and carries it out to meet the resident's needs.
Completes errands on time for all areas of campus, as needed. Example: Lab deliveries or maintenance pickups in Fort Dodge
Assists SIL, SAL and LTC with shopping needs, as needed.
Effectively communicates with residents, co-workers, and Supervisors to ensure schedules are accurate for transporting consumers.
Clean and vacuum the interiors and ensure that all specialized equipment, such as wheelchair lifts, is in proper working order. Transportation driver will also wash and polish the outside of the bus to keep it clean and attractive. They may take the bus to a mechanic for routine maintenance or when the specialized wheelchair equipment doesn't function properly.
Able to collect and calculate transportation fares and vouchers from consumers.
Can safely detach the wheelchair security device, lower the patient out of the bus using the lift, and wheel the patient to his/her destination.
Completes in-service training and MedTrainer courses in a timely manner.
Responsible for keeping active Chauffeur's License (class D) license by following all DOT requirements.
Performs various ground keeping and maintenance tasks/repairs as requested.
Knowledge, Skills, and Abilities
Knowledge of a patient centered model of service delivery supporting the mission, vision, and values of Humboldt County Memorial Hospital.
Certified in First Aid and CPR
Abide by HCMH Standards of Behaviors and Key Service Behaviors.
