We are seeking an After-Hours Licensed Vocational Nurse (LVN) that will work under the supervision of a Registered Nurse to provide skilled patient care services as outlined in the written plan of care. MIUST BE BILINGUAL IN CANTONESE Schedule: Tuesday, Wednesday & Thursday: 4:00pm - 12:30am Saturday & Sunday; 2:00pm - 10:30pm Location: San Francisco County and Marin County as needed By the Bay Health follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include (but are not limited to): Assist a registered nurse in providing direct care Initiate communication with physicians and managers, IPG members, HHA's and other health care providers as needed to facilitate optimal care Instruct primary caregivers, patients, volunteers and employed caregivers in care provision based on the Plan of Care and as requested by the Primary Nurse or RN Manager. Note: The LVN receives direction from the RN on-site and may not alter the patient/family plan of care Attend patient conference and participate in discussion of issues to facilitate optimal care Education/Experience: Completion of a Licensed Vocational Nursing Education Program from an accredited school Minimum of one year of LVN experience preferably in an acute care setting.
