We are seeking a compassionate and experienced Advanced Illness Management (AIM) Palliative Registered Nurse to join our team. Reporting to the Director of Palliative Care, this role provides high-quality palliative care to patients in their homes, assisted living facilities, and skilled nursing facilities. The AIM Palliative RN collaborates with patients, families, and interdisciplinary team members to assess, educate, and coordinate care while ensuring comfort and quality of life.
Essential Functions of Position:
Conduct in-person and telephonic RN assessments and screenings for palliative care patients.
Complete Risk Stratification Assessments to identify patient support needs in consultation with the Nurse Practitioner.
Observe patient visit frequency based on individual needs and risk identification.
Educate patients and families on disease processes, self-care, end-of-life care, medications, nutrition, and treatments.
Communicate findings to the Palliative Nurse Practitioner and/or PCP, including changes in condition or signs of decline.
Coordinate care with palliative team members and support hospice referral as appropriate.
Document timely clinical records including assessments, progress notes, care plans, physician orders, and treatments.
Support care plan development from initial assessment through discharge, addressing functional decline, symptom management, nutrition, cognition, and family/caregiver needs.
Collaborate on modifications to individualized palliative plans of care.
Facilitate care delivery and obtain additional resources as needed in accordance with the Plan of Care.
Participate in evening/weekend on-call rotation as required.
Facilitate prescription transmission to pharmacies, manage lab and diagnostic results, and request durable medical equipment as medically necessary.
Maintain effective relationships with supportive care team members and other healthcare providers.
Communicate and document clinical findings, treatment plans, and care provided in accordance with nursing standards and regulatory requirements.
Adhere to company policies, HIPAA privacy standards, business ethics, and mandatory compliance programs.
Implement marketing and promotional initiatives as directed.
Perform other related duties as assigned.
About You:
Required Qualifications:
Active and unrestricted Registered Nurse (RN) license with no board actions
Minimum 2 years of clinical RN experience, preferably in skilled nursing, home health, palliative care, hospice, geriatrics, or oncology
Demonstrated ability to provide quality care in high-empathy environments
Proficiency in Microsoft Outlook, Word, Excel, and electronic medical records
Exceptional communication, time management, and critical thinking skills
Keen observation skills with the ability to identify changes in condition and escalate appropriately
Comfortable working independently in the field and within interdisciplinary care models
Valid driver's license and reliable transportation (field-based role)
Preferred Experience:
Previous experience in palliative care, hospice, or oncology nursing
Familiarity with risk stratification, symptom management, and home-based care planning
Experience working in or with assisted living and skilled nursing facilities

Welcome to the Grow Your Own Program! This program provides a unique opportunity for current GRPS employees to earn a teaching credential and become a teacher at zero cost to the employee. Please read the information below carefully, as it outlines the program requirements, expectations, and next steps.
Program Overview:
The Grow Your Own Program is designed to support current district employees who are interested in becoming certified teachers. If selected for the program, you will have the opportunity to:
Earn your teaching credential through an accelerated graduate program offered by Davenport University, which we will fully fund on your behalf.
Continue working in your current district position (full-time, part-time, temporary, substitute, or contract) while completing the graduate program.
Receive academic and professional support from the district and the University throughout your journey to becoming a teacher
Make an impact on your community and become the teacher our scholars need!
Program Requirements:
To participate in the Grow Your Own Program, you must meet the following criteria:
Bachelor's Degree: You must have earned a bachelor's degree from an accredited institution.
Good Academic Standing: You should have a cumulative GPA of 3.0 or higher. If your GPA is below 3.0, you may still apply, but we will evaluate your academic history on a case-by-case basis.
Good Performance in Your Current Role: You must be in good standing with the district in terms of attendance, performance, and behavior. This includes a positive track record in your current role at the district.
Program Commitment: You must be able to commit to completing the accelerated graduate program, which is intensive and requires significant time and effort to balance work and study. It will require a high level of academic dedication and the ability to manage your responsibilities effectively.
Post-program Commitment: Program participants will commit to working in a GRPS teaching position for at least 2 years upon successful completion of the program
Application Process:
Step 1: Complete this Interest Form
By completing this form, you are expressing your interest in applying for the program. This is not an official application, but it is the first step in the process.
Step 2: Internal GRPS Vetting
After submitting the interest form, the HR and Talent Acquisition team will review your qualifications and performance to determine whether you meet the program's requirements. You may be contacted for a brief interview or additional information.
Step 3: University Application
If you are selected by GRPS, you will be asked to apply to Davenport's Urban Education/Alternative Route to Teacher Certification graduate program. GRPS will assist you with this application process and provide a recommendation from the district based on your internal application and performance.
Step 4: Final Selection
Once you are admitted to the university program, you will be officially enrolled in the Grow Your Own Program and will begin your coursework and teacher training.
Program Expectations:
Commitment to Complete the Program: The graduate program is intensive and accelerated. You must be prepared for a significant commitment of time and energy.
Work While Studying: All classes are online, but you will need to continue working in your district role while completing the graduate program, so it is important that you are able to manage your time effectively.
Ongoing Support: Both the district and the University will provide support throughout the program, including mentoring and professional development. The district will also provide support with your graduate application and admissions process if needed.
Certification and Employment: Upon successful completion of the GYO program and teacher certification process, you will be required to commit to 2 years of service with the district (preferably at a CSI, ATS, or TSI school) in exchange for receiving funding for the program. Details of this commitment will be provided before enrollment.

Helps the patient to understand what to expect related to receiving a bill if initial check shows that Center for Medicare and Medicaid Services (CMS) will not reimburse the hospital for the ordered testing.
Understands and communicates when the specimen needs to be collected within a specific time frame or when the patient is fasting.
Demonstrates ability to identify a compliant diagnostic order. Knows how to contact the physician if a compliant order is needed or if edits need to be made to an existing order.
