The primary purpose of the Psychiatric Rehabilitation Service Coordinator is to implement the programs of the Social Services Department, to assure that the medically related emotional and social needs of the resident are met/maintained on as individual basis, to safeguard the health, safety and welfare of all manner in accordance with facility's established policies and procedures applicable laws and regulations and the directions of your supervisors, who include the PRSD, administrator and/or other members of the community's management to whom such persons report, in order to assure that the highest level of social services for the community's residents is maintained at all times. As a PRC, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
* To provide the resident with a stable therapeutic relationship.
* To orient the resident to the community.
* To review and assist the resident in understanding his/her treatment plan and program schedule i.e., day program, in-house groups, and activities.
* To prepare and assist the resident with active participation in his/her treatment plan review.
* To provide and/or coordinate the delivery of the psychiatric rehabilitation services programs.
* To monitor the resident in the areas of self-directed care and for overall compliance with their treatment plan.
* Completing proper documentation upon admission, quarterly, and annually for assigned residents.
* Documenting the resident's progress on a regular basis, including incidental, monthly, and quarterly notes.
* Contacting community agencies for the purpose of resident referral.
* Maintaining knowledge of residents on psychotropic medications, including the identified purpose of a specific medication
* Ensure resident participates in behavioral programming to help the resident achieve his/her highest functional level.
* Coordinating, assigning, and monitoring group and individual counseling sessions for each resident.
* Being familiar with the facility policies on behavioral emergencies and be trained in crisis management and diffusing a potentially volatile situation, especially concerning aggressive/agitated behavior.
* Participate in educational trainings and in-services provided by the community.
* All other duties as assigned.
EDUCATIONAL AND ESPERIENCE REQUIREMENTS
* Bachelor's degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology) required.
* Possess a minimum of one year of supervised experience in mental health or human services.
