Essential Functions: Complete comprehensive psychosocial assessments with potential consumers and families to determine program eligibility (both programs) and placement needs (Dom Care only). Develops and maintains an individualized service plan (ISP) required for consumer, and oversees that services are being utilized. Conducts follow-up visits at prescribed intervals, maintaining regular contact with consumer and provider to ensure mutual satisfaction within program guidelines. Provides assistance and counseling to consumers, providers and families ensuring service delivery and social and emotional support and identifying other community resources that consumers may be able to utilize.
