More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today. CAMBA's Ending the Epidemic Program aims to reduce transmission of HIV among populations at risk in Brooklyn. The program will focus on men who have sex with men (MSM) and transgender women of color and will reach out to all populations at high risk in the borough, including the following: Black and Latine MSM; individuals engaging in transactional sex; persons with HIV-positive partners; and Black and Latine men and women in high prevalence neighborhoods. The program will include two components: an online targeted outreach campaign to reach out to clients and a new media campaign. Both seek to increase awareness of combination prevention - especially biomedical interventions like pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) - and recruit clients into a multi-pronged intervention. The intervention will consist of the following components: online targeted outreach activities through engaging targeted individuals in risk assessment and a brief intervention; delivering Combination Prevention Education sessions addressing PrEP/PEP, consistent condom use, risk reduction practices, and the importance of regular STI and HIV testing; conducting linkages to services, including referrals to, other CAMBA HIV prevention programs, clinical providers of PrEP/PEP, in-person and at-home HIV testing, as well as social services; benefits navigation; and follow-up with clients who do not link to PrEP/PEP services Position: Client Advocate Reports To: Assistant Program Manager Location: 1259 Flatbush Avenue Brooklyn, NY 11226 What The Client Advocate Does: Recruit clients into multi-pronged interventions consisting of sexual health and social services support Recruit gay and non-gay identified young men who have sex with men (MSM) And Trans & Non-Binary People, of color for group and individual HIV/STD/Hepatitis C testing, PEP and PrEP services and risk reduction counseling. Promote the program and deliver HIV Prevention Biomedical Interventions to clients through gay hook-up apps (e.g., Grindr, Jack'd, Hornet, Adam4Adam) social media. Assist program staff to facilitate intakes. Create and post content for social marketing campaigns addressing HIV prevention and other health concerns for MSMoC. Conduct outreach activities at high-risk venues to market the program to recruit new clients and re-engage former clients. Develop action plans with clients enrolled in navigation or support services Follow-up with clients and with referral organizations regarding client contact and progress with referral organization. In collaboration with program staff, work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency. Contact clients via telephone, mail and/or face-to-face home and office visits to ensure adherence to program, and document via progress notes. Escort clients to appointments (educational, medical, social service, etc.). Provide all required information for weekly/monthly/quarterly/annual reports. Create and maintain client files and make copies of all client documents. Prescreen clients over the telephone for eligibility and may schedule intake appointments. Assist clients in completing all CAMBA intake applications and forms. Schedule appointments for client with referral organizations. Input client data and client progress information into automated database. Participate in evaluation activities, such as conducting surveys with support group and event participants. Other duties as assigned. Minimum Education/Experience Required: Sufficient education (English-speaking, reading, writing, & math abilities) to comprehend oral and written information and to assist callers and visitors and staff. Other Requirements: Two (2) years of relevant experience, preferably in a mid-sized (250+ employees) organization. Good written & verbal communication. Computer literacy in Microsoft Office Suite. Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues. Experience and/or comfort conducting online outreach via social media and dating/”hook-up” apps and websites. Required to work occasional evenings/weekend days Bi-lingual English/Spanish (Preferred) Compensation: $43,000 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. Homeless (often mentally-ill and substance-abusing) men and women are provided with comprehensive services to stabilize their condition in order to successfully transition into to permanent and/or supported housing. CAMBA's Magnolia House Women's Shelter, located in the Brownsville section of Brooklyn, serves 200 single women at any given time. The majority of these clients are diagnosed with mental illness, some dually with chemical addictions as well. Position: Client Advocate Reports To: Clinical Supervisor Location: 1424 Herkimer Street, Brooklyn, NY 11233 What the Client Advocate Does: Assist program staff by being a client liaison with outside organizations regarding such matters as housing, education, healthcare, social services, legal issues, etc. Assist clients in attaining their goals by identifying and locating community resources and by referring clients to appropriate services both within and outside CAMBA. Plan, coordinate and facilitate social/peer support events, including group facilitation for clients to promote wellness and recovery. Program Description: Goddard Riverside Community Center (GRCC) Learning to Work (LTW) for Transfer Schools Program at Edward Reynolds West Side High School provides students over-aged and under-credited ages 16 -21 with services and opportunities to foster engagement through academic and personal development that lead to earning a high school diploma. Primary goals include getting students to graduate with realistic and meaningful post-secondary plans, gain knowledge and skills that will increase the employability and awareness of post-high school opportunities available to them. Some opportunities include higher education, trade/technical schools and community service programs. Using the Primary Person Model, each student has an LTW Staff Member as a support to minimize and/or remove barriers toward graduation including engaging students in college preparation, and meaningful career exploration including subsidized internships, academic and personal support services to increase self-direction and self-sufficiency. Position Summary: The LTW Advocate Counselor is the primary person support for a cohort of students who he/she knows well, counsels to promote goal-setting and supports towards the achievement of graduation with a meaningful postsecondary plan The Advocate Counselor will be instrumental in working alongside students to break through barriers that prevent them from achieving academic, social and emotional success. Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, etc. Monitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via progress notes. Recommend closing of cases in which clients have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area). Provide all required information for weekly/monthly/quarterly/annual reports. May conduct client's and/or families' periodic re-assessments. May schedule appointments for client with referral organizations. May escort clients to appointments (educational, medical, social service, etc.). May assist clients in completing applications for benefits and entitlements. Minimum Education/Experience Required: Bachelor's Degree (B.A.) and/or equivalent experience. Other Requirements: Equally at home conversing with gang members, community activists, law enforcement personnel, and public officials. Subject to a criminal background check and random drug testing. No pending criminal cases or prior convictions for sexual assault, child abuse or domestic violence or history thereof. Strong knowledge of the Brownsville community. Familiarity with Brownsville 73 rd Police Precinct community challenges of teens and young adults there; strongly preferred. Ability to work a flexible schedule including morning, evening and weekends as needed. Must have understanding of, sensitivity and commitment to working with families of youth that have experienced gun related trauma. Case Managers should be prepared for various outdoor weather conditions during their regular workday. Compensation: $50,000 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization. As a Family Support Specialist, you will serve as a compassionate advocate for clients in transitional housing, offering supports to overcome obstacles and navigate systems for obtaining permanent housing. Teamwork • Attend all scheduled meetings • Collaborate with the team to resolve challenges and remove barriers. • Participate in team meetings, trainings, and professional development activities • Support unit inspections and collaborate with security staff to maintain a safe environment. • Document and help maintain l records for families. Essential Functions: • Ability to navigate stairs in the assigned building. • Escort clients to appointments, including apartment viewings. Qualifications: • High school diploma, GED, or equivalent. • Previous experience in social services, supportive housing, or emergency shelter settings. • Strong interpersonal skills and ability to build rapport with diverse populations. • Proficiency in computer use for documentation and reporting. • Excellent communication and organizational skills. • Compassion, empathy, and a genuine desire to help others. • Bilingual proficiency in English and Spanish (preferred).
