The Manager of the Clinical Resource Department is responsible for the day to day operations and clinical management of the Case Management, Utilization Review and Social Work, with a secondary focus on the Clinical Documentation team. This position will oversee functions of the Clinical Resource Department as they pertain to discharge planning, utilization management, and care coordination/high risk patient process while maintaining the integrity of daily operations. This position is responsible for developing and conducting ongoing education and process improvement for Case Management/Social Work staff.
PRIMARY DUTIES AND RESPONSIBILITIES:
Day to day management of the case management and social services program staff and workflows, including scheduling and performance evaluations. Assists the Manager of Operations with the Clinical Documentation team where necessary
Rounds on patient units on a regular basis to identify problem areas and assist in the facilitation of the discharge process
Ongoing development and completion of continuing education programs for Clinical Resources Management staff, including case managers and social workers specifically related to role functions and implementation of new processes.
Provides ongoing program education for new staff, including new Case Managers, the Trainee program hires, social workers, physicians, nurses and allied health professionals as it relates to discharge planning and utilization review.
Tracks and trends Clinical Resource Management program compliance. Creates a dashboard to see where opportunities for improvement are and implements process improvement activities around areas identified.
Develop, mentor and coach continual improvement in the collaborative practice between physicians, nurses and clinical resource team, and within the clinical resource department itself
Analysis and review of program metrics using working knowledge of the systems used in the departments.
Stays current with regulatory changes in the case management field.
Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization's culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.
EDUCATION/EXPERIENCE/TRAINING:
Bachelor level degree in Nursing required.
Master's Degree preferred
Current Massachusetts Registered Nurse license
Certification in Case Management or Clinical Documentation Integrity preferred
Two years' experience in process improvement
Minimum 5 years Case Management experience in an acute setting.
Experience in development and presentation of professional continuing education programs
Knowledge of CMS regulations
Excellent computer skills - familiarity with Windows based software programs
Organizational, analytical, writing and interpersonal skills
Management/supervisory experience preferred

1. The Opportunity

* Collaborate with interdisciplinary care teams (ICT)-including LTSC, GSSC, primary care providers, specialists, and behavioral health treatment facilities-to support program enhancements, process improvements, care coordination, discharge planning, and aftercare.
* Participate actively in ICT meetings, contributing insights on behavioral health concerns, treatment options, care strategies, and engaging in collaborative decision-making regarding enrollees' care.
* Receive and triage behavioral health and substance use referrals, coordinating with the ICT and primary care providers to refer enrollees to appropriate inpatient or outpatient services.
* Develop, update, and implement enrollee-centered care plans in collaboration with enrollees and care teams, promoting self-care, shared decision-making, and addressing identified needs through routine engagement and proactive outreach.
* Conduct outreach, assessments, and home visits via telephonic, electronic, or in-person contact for identified enrollees, including routine evaluations and post-transition care.

Job Summary
The Newton-Wellesley Hospital Domestic Violence and Sexual Assault Program provides individual, community and systems responses to sexual assault and domestic violence through direct advocacy services, education, community linkages, quality improvement and program evaluation. · Provide direct services to primary and secondary survivors of sexual assault, domestic violence, and stalking. Services include but are not limited to crisis intervention, collaborative safety planning, advocacy, short and in some instances long-term counseling, as well as the provision of information, and supported referrals. · Holistically and organically assess the particular needs of each survivor and ensure that appropriate services are offered to the survivor and any safe family and friends, including children, elders and animal companions. · Provide on-call consultation to hospital and community-based health care providers. · Develop and co-facilitate support, empowerment and wellness groups for survivors of interpersonal violence. · Provide outreach in the hospital and in the community to increase awareness of the availability of services for survivors. Work to redress any barriers to obtaining such services. · Work closely with program staff and volunteers to provide trauma-informed and survivor-centered services. · Work collaboratively with providers both within and outside the hospital to ensure an anti-oppression approach to the challenges faced by survivors both individually and collectively.

Qualifications
Must have clinical experience and grounding. Experience in crisis and trauma settings, where significant, can fulfill clinical grounding requirement. Clinical experience can also be proven through licensure in psychology, social work, or mental health counseling, however, such licensure is not strictly necessary. · Must have 3-5 years of experience in sexual or domestic violence organization. Experience in similar fields (human trafficking organizations, hate crimes, child abuse) will also suffice. Licenses, Certifications, and/or Registrations -- Master's degree in Social Work, LICSW required. - Must be DPH-certified as a rape crisis counselor or have the willingness to be so trained. - Must have appropriate training as a domestic violence advocate or the willingness to be so trained. The Employee Assistance Program (EAP) Counselor supports the mission of the EAP and provides employee assistance services to staff, employees, and their immediate family members across Mass General Brigham, as well as to employees of all affiliated client companies that have arranged for EAP services.
