Perform the intake process, including gathering the necessary documentation to determine functional eligibility (demographics, PMIP, supporting documentation, medical records)

Complete an annual ULTC assessment to determine ongoing functional eligibility for waiver services.
Actively guide the Service planning process to assist clients and families in identifying needs and necessary services and supports
Make referrals to providers to set up Medicaid waiver services or other necessary services
Monthly contacts with each client on the caseload (either directly or through partnering agencies)
A quarterly thorough review of services that the client is using
Annual IN-HOME visit to the clients' residences to observe and document all needed services and supports and obtain all required signature pages.
Support Clients and families to find and interview service agencies to determine if they can meet their identified needs.
Make and follow up with timely referrals to other necessary services
Maintain an accurate and up-to-date record on state-designated information systems (BUS, Bridge, DDD Web), etc.
Complete and thoroughly document regular reviews of clients individualized plans and make necessary revisions
Provide copies of the service plan, PAR, and other supporting documentation to the client, families, and providers promptly
Set up IDT meetings with the client, provider, and other necessary attendees when issues arise that need to be discussed or changed
Thoroughly document concerns that a client may have, the follow-up and additional monitoring
Monitor that services and supports are being provided as documented in the individual plan through a variety of means including but not limited to: attendance at site meetings, attendance at provider meetings, visits to the clients home during support hours, paper reviews, and telephone calls to clients and providers
Document all billable supports provided by CM's by entering a log note in the BUS
Maintain current and updated information on all clients on assigned caseloads
Maintain the client's master file and ensure accuracy of the information, proper content, and security.
Provide resource coordination and/or referral for supports for SSI, SSA, Medicare, Medicaid, natural supports, and third party resources
Develop and maintain good working relationships with other community agencies, service delivery agencies, referral sources, etc., and their staff to be knowledgeable about community resources and an effective client representative and advocate.
Provide notification of clients' rights and appeal rights (803)
Provide notification as necessary, and assist in conflict or dispute resolution and the appeal process
Complete critical incidents for clients promptly as identified by HCPF
Complete all necessary Critical Incident follow up promptly
Coordinate and cooperate with the supervisor, agency director, and other directors
Light lifting of files, records, and/or office materials may be necessary at times
Be able to sit and use a computer for a full shift
Perform other reasonable duties as assigned.
