Job Functions: Provide safe and efficient transport of patients in the ambulette as scheduled by dispatch. Operate/drive ambulette in accordance with company's driving standards, policies and procedures. Ensure safety of bus occupants by observing safe driving practices; seat belt application and general patient security. Record transportation logs; obtain patient documentation; complete all forms are required by policy. Conduct regular ambulette inventory and equipment testing to ensure proper operation. Report and document all needed repaires to proper maintenance department and management. Comply with all FCC regulations and company policy when utilizing electronic communications equipment. Order supplies and equipment as needed from purchasing. Perform general safety and proper operations check; includes but is not limited to: fluid checks, tire checks, horn check, fuel checks. Maintain ambulette; includes but is not limited to: washing & waxing exterior; cleaning interior; re-fueling, adding air to tires, checking oil. Provide basic life support-first aid or CPR in emergency situations to patients. Attend all mandatory staff meetings as scheduled. Family Health Services is a Federally Qualified Health Center (FQHC) that provides quality, affordable primary care, dental, and behavioral health services. Our "high touch, high tech" clinics offer unique patient focused care to anyone in the community, regardless of their ability to pay. We are rapidly growing in Sandusky, Ohio at our two locations as well as in Norwalk, Ohio and surrounding areas. We also offer behavioral health and primary care at several local schools in hopes of eliminating barriers to those needing services. We are seeking a reliable part-time Van Driver. The driver will operate FHS vans which are equipped to provide nonemergency transport of mobility-impaired individuals by accommodating wheelchairs, walkers, and strollers and offering safe custom transportation to FHS medical/dental/behavioral health appointments and treatments. Essential Functions: * Picks up patients at home or other facility. Provides the assistance required tosafely board patients and caregivers. * Ensures wheelchairs and passengers are properly secured before starting van.Depending on route, may pick up several patients for a common destination. * On arrival at FHS site, assists patients in exiting the van and reaching the properoffices or treatment areas. * Checks the van to ensure that no personal items are left behind. * Provides first-aid or CPR if required during patient transport. Immediately notifiesemergency staff and dispatcher if skilled assistance is required. * Completes reports at the end of a shift. * Immediately reports traffic accidents to dispatcher, requesting police or ambulanceservice as needed. Assists injured persons from first-aid kit until these servicesarrive. Completes company accident reports. Competencies: * Possession of the patience and tact necessary for contacts with patients, bothadult and children, and their caregivers. * Must be reliable and capable of working independently and remaining calm in caseof emergency. Physical Demands: * Ability to lift up to 50 pounds, push, pull, and position patients frequently. * Ability to perform considerable reaching, stooping, and bending. * Hearing and vision abilities within normal range (corrected) to drive safely and toobserve and communicate with patients. * Regular exposure to injuries and illness. * Frequent need to handle emergency or crisis situations. * Exposure to a variety of weather conditions. * Occasional exposure to the risk of bloodborne diseases. * Be able to withstand frequent walking, handle opening doors, and operating awheel chair lift. Position Type/Expected Hours of Work: This is a part-time position with potential to full-time as needed, and hours of work and days are Monday through Friday 8:00 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties demand.
