About the Position
The Kinship Home Finder Specialist will search for and identify maternal and paternal relatives and fictive resources for children placed in non-kinship homes in our Community foster care and TFFC programs, within 30 days of placement with Graham using the Family Finding Model. Work in collaboration with case planners to engage and support kinship families through coaching and facilitation of support groups specifically for Kinship resources.
Identify kinship resources for children in non-kinship placements. Build relationship by having regular contact with kin/fictive resources to develop openness and trust. Builds a network with persons close to and knowledgeable about the child, and maintain regular and on-going contact with them.
Engage in recruiting Kinship foster parents, by utilizing data base, social media, etc.
Provides family search efforts by implementing the process of identifying, locating, and contacting persons with whom the child already has, or had, a bond or positive relationship with, with the knowledge and approval of the child's caseworker.
Prepares and submits reports, calendars and other deliverables as required by agency supervisor, NYC Children's Services and/or OCFS. Assures scheduling, monitoring and tracking of all possible kinship resources.
Facilitate and scheduled 30 day conference with case planning team to discuss the outcome or barriers in identifying kinships. Tracks all identified kinship resources until placement. Actively participate in the Initial placement conference for non-kinship cases

They will advocate for the provision of therapeutic services to all children in Foster Care, which meet optimal standards for clinical excellence.
Duties and Responsibilities:
Provides clinical and administrative supervision to a unit of Nursing and/or Clinical Care Coordinators to monitor performance, provide training to teach new skills and maintain strong workflow practices, coaches to improve performance outcomes and promotes staff self-care/wellness
Assists in the planning and implementation of monthly program management tools to promote continuous feedback about operations and to develop best practices and support department outcomes.

Promote evidence-based wellness, prevention, and treatment practices for a variety of health and behavioral health disorders in order to achieve optimal outcomes for children/adolescents in foster care

Position Overview
The Preventive Director in partnership with the Associate Vice-President of Social Services, alongside the Preventive management team will insure that the multi-site and multi-method programs have a consistently strong approach to partnering with families to promote the safety and wellbeing of all children; insure that the agency practice approach (Solution-Based Casework) is used to partner with families to help develop and encourage families to build connections to positive supports in their community who will help them sustain their progress; collaborate effectively with all internal and external programs to insure that our families and staff have access to high-quality programs and resources that meet their needs. Manage the day to day function of the Community-Based General and Intensive Preventive Service Programs according to New York State, New York City Administration of Children mandates and in accordance with the Agency's Pillars (Treat all with respect; Promote Safety and Well-Being; Encourage learning; Support opportunities for growth and Thrive in life)so as to strengthen families and protect children. Engage in community relation, collaborations, linkages and other activities that support the program, assist the community and aid families.
MAJOR RESPONSIBILITIES (but not limited to):
Drives the implementation of practices, policies and procedures consistent with Graham Windham's Prevention Program Plan for Performance Excellence.

Department: Family and Community and Support Services
The Graham SLAM program works with youth between the 9th grade and age 26 who are in Graham Windham's programs including foster care, foster care prevention and community-based afterschool programs. The program aims to put youth on a livable wage path by focusing on their success in high school, college and career development. The program ‘sticks with' youth through age 26, or until they are on a living wage career path. SLAM Coaches are focused on providing social and emotional support, goal setting and goal tracking. A Coach and youth work on setting key program goals in the areas of education and career, with the purpose of getting them on a livable wage pathway. Coaches will also work with their youth setting and tracking customized goals in the areas of wellness and independent living skills. Examples include learning how to budget, opening a bank account and time management. There are ongoing, individual support and planning sessions where a SLAM Coach will support the youth's educational and career goals.
Graham SLAM Coaches will:
1. Work one-on-one with each young person on their coach load to create individual goals. Uses Solution-Base Casework and Motivational Interviewing techniques to develop and follow action plans to reach these goals, with consistent support, communication, and follow-through.
2.Use SLAM's goal setting tools to develop customized action plans for each youth to help them reach their goals.
3. Establish strong relationships with adolescents using effective modeling of pro-social skills.
4. Work collaboratively with other system providers in the youth's life including to help ensure that the varied systems are driving towards similar goals in a coordinated fashion.
5. Meet regularly with parents/foster parents/adult supports to plan with them for youth success, and to support them in helping the youth in their care, reach their goals.

9. Connect youth to resources in their community.
10. Help students select vocational programs or colleges based on academic, social, and financial needs.

About the Internship
Youth Success Intern supports students in Graham After School Programs and advocate for their emotional, mental and physical wellbeing. This internship will allow the selected candidates to connect and mentor at-risk youth through the program with helpful resources. Each intern will be expected to conduct home visits to further engage and support the youths through field work.
The Youth Success Internship allows selected candidates to work at Graham's Harlem Beacon Community Center in PS/MS 123 a K-8 school. The intern will collaborate with program staff to identify students that are struggling emotionally, socially or academically and identify the root cause of their issues. They educate students on social and emotional concepts and they will administer mental health support to the student body.
Duties and Responsibilities
In the Youth Success Social Work Intern include but are not limited to:
Promoting the general well-being of students
Offering mentoring and suggestions for Program Staff
Developing and providing crisis intervention strategies to improve student success
Working to maximize coping skills for students in difficult situations

The Role: DDI's Day Program Manager works with program leadership and Direct Support Professionals to ensure that our day services are meeting the needs of the people we support.
What You'll Do:
Supervise implementation of Individual Program Plans.
Ensure active treatment in assigned programs, data collection, monthly tally and organization of data for QIDP.
Active participation in program manager meetings, behavior review meetings when appropriate and meetings with clinical team members when needed.
Conduct weekly/monthly full staff meetings.
Ensure the completion of individual-related program materials.
Promote staff development, which includes but is not limited to performance improvement initiatives including training, mentoring, reviews, and any necessary corrective action plans.
Incident reports (internal and reportable)
Ensure completion of SUB's
Ensure full documentation of SCIP procedures.
Investigation and documentation of injuries of unknown origin.
Day to day supervision and assigns work daily to Instructors and Assistant Managers to program areas and tasks. This includes scheduling staff breaks while providing the most optimum coverage.
As needed contact with individuals' families, guardians, residential caretaker's providers, and Care Managers.
Attend Life Plan meetings and create Staff Action Plans.
As needed will be in ratio.
Review and approve staff schedules and timecards.
Report maintenance needs or unsafe conditions to Maintenance Department or Supervisor.
What You Bring To DDI:
High School Diploma
1 year of hands on teaching/instruction of developmentally disabled individuals
1 year of OPWDD supervisory experience
Working knowledge of OPWDD regulations
Excellent interpersonal skills
Valid NYS driver's license with an ability to become approved to drive an agency vehicle.
