What You'll Do:
As a Home Care Supervisor, you'll be the clinical leader, mentor, and advocate for patients, caregivers, and the community. Your responsibilities will include:
Patient Care & Advocacy
Initial Contact & Onboarding: Meet new patients, explain services, and complete start-of-care assessments to ensure smooth onboarding.
Individualized Care Plans: Develop tailored plans of care that address medical needs, personal preferences, and family involvement.
Ongoing Monitoring: Track patient progress, adjust care plans as needed, and report changes in condition to physicians and care teams.
Family Support: Teach or supervise household members assisting with patient care, ensuring confidence and safety at home.
Team Leadership & Supervision
Caregiver Guidance: Instruct Home Care Nurses, Aides, and Personal Care staff on care plan details, expectations, and best practices.
Performance Oversight: Review caregiver documentation, monitor reliability, and provide constructive feedback to ensure accountability.
In-Home Supervision: Conduct supervisory visits while staff are delivering services, ensuring compliance and quality care.
Orientation & Training: Lead or oversee training sessions for new staff, including safety protocols, clinical standards, and company values.
Compliance & Quality Assurance
Regulatory Knowledge: Ensure adherence to OSHA, DMAS, CCC Plus Program, and state licensure regulations.
Documentation Excellence: Prepare and coordinate clinical notes, progress reports, and compliance records.
Quality Improvement: Participate in interdisciplinary team meetings and quality reviews to continuously elevate patient care.
Community & Growth
Partnership Building: Establish and maintain relationships with referral sources, community partners, and healthcare providers.
Advocacy in Action: Represent Care Advantage in the community, promoting our mission of Compassion, Attitude, Respect, and Excellence.
Support Expansion: Assist with organizational growth during mergers and acquisitions, ensuring smooth integration of care standards.
Requirements
Active RN license in Virginia (compact/multi-state accepted)
Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable)
Associate's Degree in Nursing or higher
Strong communication and presentation skills
Ability to work collaboratively across teams
Reliable transportation for home visits (mileage reimbursed)
Comfortable with technology (computer/iPad documentation)
Willingness to travel regularly within assigned area

RegisteredNurse (RN) - CVICU/ICU (Critical Care) - Memorial Regional Medical Center
CVICU- We are an 11 bed Cardiac Surgery unit that utilizes a universal bed model. This means that our patients are admitted to us for their surgery and will stay until ready for discharge. This creates a wonderful opportunity for improved patient experience and safety, while also providing the nurse satisfaction of watching your patient progress from admission to discharge. We use a multi-disciplinary and patient centered approach to provide comprehensive cardiac surgery care for our patients. Our small unit size and patient centered focus has also created culture of collaboration and inclusion between the nursing staff and the cardiac surgery team. We work hand in hand to ensure that our patients receive the highest level of care. Besides our cardiac surgery patients, we also care for other ICU level cardiac patients like cardiac arrest, cardiogenic shock, STEMI, and post PCI to name a few.
Job Summary:
The ICU (Critical Care) RegisteredNurse (RN) position is responsible for delivering exceptional nursing care to patients by:
Utilizing strong organizational and leadership skills to assess patient status
Determining the priority of the patient's problems and needs
Implementing clinical nursing interventions to acute, critical patients that require intensive medical care
Developing and performing patient care plans to enhance and improve outcomes
Ensuring a safe and therapeutic environment
Essential Functions:
Collaborate with the interdisciplinary care team
Conducts and analyzes initial and ongoing patient assessment, creates a plan of care, implements treatment, and evaluates treatment effectiveness
Synthesizes and prioritizes information in order to take immediate and decisive evidence-based, patient-focused action using clinical judgement and clinical inquiry.
Anticipates and responds with confidence and adapts to rapidly changing patient conditions
Administer medications in a safe manner consistent with the State of Practice and the organization's policies and procedures

Job Summary:
The RN Clinical Float Pool assesses, analyzes, plans, implements and evaluates patient care following established patient care standards for the organization including those for patient assessment and transfer. The Med-Surg RegisteredNurse (RN) position is responsible for delivering exceptional nursing care to patients by:

The Obstetrics (OB) RegisteredNurse (RN) position is responsible for delivering exceptional nursing care to patients by:

Implementing clinical nursing interventions to acute, critical patients that require intensive medical care
Developing and performing patient care plans to enhance and improve outcomes

Essential Functions:
Collaborate with the interdisciplinary care team
Addresses the care provided to healthcare consumers in the context of woman-, newborn-, and family- centered care; and includes those who do not self-identify as women, or exclusively as women, as well as all people parenting newborns
Provides episodic (outpatient) and inpatient (at designated units) Obstetrical/Women's Health nursing care where applicable
Applies the nursing process in the care of women during pregnancy, birth, postpartum, gynecological, and other women's health issues and the neonate from birth until discharge with differing gestational ages, diagnoses, and complications
Delivers care to women of all ages requiring triage, prioritization, assessment, education, stabilization, resuscitation, and emergency management
Administer medications in a safe manner consistent with the State of Practice and the organization's policies and procedures

Education:
Graduate of an accredited Associates degree or Bachelors degree in Nursing
Certifications:
Current state licensure as a RegisteredNurse (RN)

What You'll Do:
