A DAY IN THE LIFE OF An RN
Makes initial contact with the patients and explains services offered
Completes start of care assessments and documentation to onboard new patients
Designs an individualized plan of care for each patient
Instructs assigned care team members on the needs and care plan details
Reports progress of care to involved parties
Follows up and maintains relationships with clients, always advocating on their behalf
Establishes relationships with community partners and referral sources
Keeps Clinical Leadership and Office Administrator abreast of patient condition and/or medical changes
Reviews caregiver documentation
Takes part in interdisciplinary team meetings to ensure needs of all patients are met

Requirements:
Must possess an active Virginia Registered Nurse license
Minimum of 1 year in a Nursing role- LPN to RN acceptable

The Home Care Supervisor is under the direct supervision and direction of the Regional Director of Nursing, as well as the Home Care Director if applicable for branch/region. Essential Job Responsibilities:
Makes initial contact with the patients and explains services offered

Follows up and maintains relationships with clients, always advocating on their behalf
Establish relationships with community partners and referral sources
Keeps Clinical and Operational abreast of patient condition and/or medical changes
Reviews of caregiver documentation
Take part in interdisciplinary team meetings to ensure needs of all patients are met
Supports organizational growth during mergers and acquisitions as needed
Follow all duties as described under the state regulations for the company's Home Care Licensure, as well as maintain standards appropriate to state/regional licensure, contracts, and OSHA. The duties of the Supervisor shall include:
Knowledge of OSHA, DMAS, CCC Plus Program, and State Licensure Regulations governing home care and nursing practice
Conduct orientation and training or supervising the orientation and training of agency employees assigned to the nursing service as directed
Assist with the assigning of RNs, LPNs, Certified Nursing Assistants/Personal Care Aides and other nursing support staff to individual patients
Assist in Safety Training and general training needs of employees as assigned
Perform initial in-home evaluation as assigned of patients requiring services of a registered nurse, licensed practical nurse, home care aide, personal care aide, or other nursing support staff
Initiate the patient's plan of care in accordance with nursing needs specified in the approved plan of treatment
Deliver or supervising the delivery of skilled nursing services
Revise patient's plan of treatment if skilled services are involved
Initiate preventative and rehabilitation nursing services through work with community partners
Prepare and coordinate clinical and progress notes on nursing services delivered
In-home supervision of nursing services delivered by licensed practical nurses, home care aides, personal care aides and other nursing support staff
If the agency contracts with individuals to deliver registered nurses, licensed practical nurses, home care aides, personal care aides, or other nursing support staff, the agency nurse supervisor shall be responsible for conducting in-home supervisory visits
In-home supervisory visits shall be made when the registered nurse, licensed practical nurse, home care aide, personal care aide, or other nursing support employees being supervised are present and delivering services to the patient
Reporting changes in the patient's medical or mental condition to the attending physician, care team as necessary, and individuals involved in the patient's care
Teach or supervise the teaching of household members who are responsible for assisting the patient with his/her nursing or personal care needs
Participate in quality improvement reviews, as requested
Other duties as assigned
Requirements:
Must possess an active state Registered Nurse license for the state they will be working in (compact/multi state license is acceptable)

Adult Day Clinical Coordinator - RN (Bilingual Korean/English)
Type: Full-Time

This role also includes completing home visits (Admissions and Recertifications) for clients who attend the Adult Daycare. It's a full-time opportunity for an experienced clinical leader who thrives on making a difference in the lives of seniors while guiding a dedicated team.

What You'll Do
In this role, you'll be the clinical leader ensuring quality care and compliance:
Ensure documentation meets state compliance standards.
Deliver outstanding customer service with daily interaction and support for seniors.
Conduct start-of-care assessments, design individualized care plans, and complete supervisory visits for clients both in the Adult Daycare and at home.
Provide clinical oversight of personal care services for Adult Day members.
Participate in interdisciplinary team meetings to align on patient needs.
Resolve complaints and incidents with professionalism and care.
Build strong relationships with community partners and referral sources.
Maintain compliance across patient files and documentation.
Take on additional responsibilities as assigned by leadership.
What We're Looking For
Active RN license in Virginia (compact multi-state license accepted).
Fluent in Korean and English - REQUIRED.
Experience in adult daycare or long-term care facility - preferred.
2-3 years of clinical experience, preferably in a facility setting.
Strong customer service background.
Excellent communication skills (verbal and written) across all organizational levels.
Familiarity with Medicaid processes - preferred.
Tech-savvy with computer/iPad skills for documentation.

A Day in the life of our Adult Daycare RN Care Manager
Responsible for the upkeep of documentation in compliance with state guidelines.
Responsible for scheduling, maintaining, and follow through for county inspections, meal reimbursement, and transportation reimbursement.
Schedules shifts for the daycare's floor staff
Assists the RN director when needed
Exemplifies customer service with consistent exposure and requests from our seniors
Tends to daycare participants when needed, including setting up doctor's appointments and interviews with MCO coordinators
Works in conjunction with the admin assistant
All other duties as assigned by leadership and job description
Requirements:
Active RN license in the state of VA (Compact multi-state license acceptable)
Bilingual Korean/English - Fluent - REQUIRED
Experience in adult daycare or LTC facility - REQUIRED
Minimum of 2-3 years in a leadership role, preferably in a facility
Customer service background
Comfortable working collaboratively in cross-functional teams
Ability to communicate effectively, both verbally and in writing, with all levels of an organization
Knowledge and experience with Medicaid preferred.
