Care associate job description

Atlantic Health System is seeking a full-time Community Health Associate for Care Coordination to work from 8:00 AM to 4:00 PM

Community Health Associates screen patients for social determinates of health needs including food insecurity, behavioral health needs, housing, healthy lifestyles.

They assist patients in setting short term goals to help their healthcare outcomes and access to care.

This Full-time position would be based in the Overlook Medical Center area.
Responsibilities
• Attend a paid training program
• Meet patients in a clinic or hospital and conduct a needs assessment, including helping patients to set health goals
• Make weekly follow-up calls and home visits to patients
• Motivate patients to meet their health goals
• Help patients with social issues like homelessness, substance abuse and hunger
• Assist patients with organizing their records, making follow-up appointments, and filling prescriptions
• Help patients fill out applications for benefits like insurance and food stamps
• Run a weekly support group for patients
• Document each patient encounter in detail
• Prepare reports and documents as needed or requested
• Attend regular team meetings
• Other duties as reasonably assigned
Qualifications
Qualifications/Requirments:
Long-time resident of a community with good knowledge of the resources of this community
• Excellent oral and written communication skills
• Ability to carry out written and oral instructions
• Ability to work independently and within a team
• Ability to exercise judgment in the application of professional services
• Comfortable with home visits and outreach
• Comfortable with computer data entry
• Bilingual in Spanish or various Indian dialects is a plus
• Valid NJ Driver's License a must
Minimum Education:
High School Diploma or equivalent required
Relevant Experience:
1-3 years of relevant experience preferred

Overview



Position Summary:
The Care Coordination Associate assists in facilitating efficient functioning of Clinical area. Responsible for coordination of the patient's visit in the clinic. Prepares, compiles, and maintains appropriate records and reports in accordance with established guidelines and procedures. Coordinates of highly complex patient visits in a high patient volume specialty with inter-professional patient care providers. Demonstrates commitment to a patient and family focused model of care.


Minimum Qualifications:
Minimum of one-year related clerical or equivalent experience in a medical setting. Experience in clinical data entry experience in medical transcription or equivalent training. Acquired knowledge in an ambulatory pediatric environment preferred. Effective verbal and written communication skills. Broad knowledge of computer software including word processing, spreadsheets and presentation applications; keyboarding skill level at 45wpm. Preferred for positions in medical/clinical areas: basic medical and scientific terminology. Bilingual preferred. [If fluent in another language, assist in interpreting on a limited basis (phone calls to families to check demo info, brief dc planning on phone, discuss learning needs/expectations). NOT for extended teaching, not to translate DC orders/instruction in KIDS.] Language proficiency testing required according to policy MHR 23.1


Education/Licensure/Certifications:
High School graduate or equivalent. Training or experience in healthcare field, familiar with health care terminology.

The Care Associate provides and documents assistance to patients with activities of daily living, comfort measures, and maintains a safe and clean environment under the direction and supervision of a Registered Nurse.

Qualifications
Education: High School or GED, Required. Licensure/Certification: Completion Novant Health sponsored training program, Required. Completion of Novant Health Safety Attendant training, Required. Additional Skills Required: Upon completion of the Novant Health sponsored training program, the Care Associate works within the guidelines of nursing and Novant Health policies and procedures. Must exhibit strong communication skills; ability to write effectively; effective computer skills, and strong customer service skills. Additional Skills Preferred: Basic computer skills for documentation. Medical Unit Receptionist (MUR) and Safety Attendant duties may be assigned as determined by unit leaders and unit needs. If needed in the MUR role, the Care Associate must be able to perform the MUR skills as indicated in the MUR job description. If needed in the Safety attendant role, the Care Associate must complete appropriate training and be able to perform the Safety Attendant skills as indicated in the Safety Attendant job description.
