Summary:
The Quality Improvement Specialist reports to the Director of Quality and Patient Experience and is an active member of the Quality and Performance Improvement Department. Responsibilities include abstraction of quality measures from electronic medical records systems using measure specifications, auditing clinical documentation based on expected best practices, and participation in improvement projects.
This position will review patient charts for completeness and accuracy that reflects services provided, evidence-based practices, current patient status, individualized care plans, progress toward goals and outcomes of care. The Quality Improvement Specialist provides coaching and findings to the interdisciplinary team members as needed.
What You'll Bring:
Registered Nurse, currently licensed in Massachusetts in good standing.
Associate's degree in nursing required; Bachelor's degree in Nursing preferred.
Minimum three years of varied clinical experience as a professional nurse, and preferably one of the three years in a hospice, home care, or long term care setting.
Understanding of hospice philosophy and principles.
Experienced with Electronic Medical Records.

Summary:
Reporting to the Director, Patient Access Services, the Director, Continuity of Care, the Manager, Continuity of Care or the Patient Access Supervisor, the Patient Access Representative (PAR) provides exceptional customer service to Care Dimensions patients, families, and referral sources. The PAR works with both active and prospective patients and can educate/explain all services offered. The PAR is responsible for answering phone calls, registering patients for all business lines, and ensuring the caller is sent to the appropriate department/team if transferring is necessary.
Qualifications:
High school diploma required, Associate's Degree preferred. Minimum one (1) to three (3) years' work experience in a health care or medical office environment; or, any equivalent combination of education and experience that provides the requisite knowledge and skills needed for the position.
Proficiency in MS Office/Excel/PowerPoint/Outlook and healthcare specific systems and databases required.

Summary:
As a member of the interdisciplinary team, the Bereavement Coordinator works under the supervision of the Manager of Bereavement Services and is responsible for day to day oversight and provision of bereavement services at the organization's branch office, assisting in the planning and providing of bereavement services, outreach and education and related services to families served by Care Dimensions.
Qualifications
Qualifications:
MA/ MSW degree from an accredited/recognized school of social work, expressive therapy, or related field. LICSW, LCSW or LMHC required or, if in progress, will be obtained within one year.
Minimum of three years' experience in the field of grief counseling preferred.
Strong interpersonal skills, oral and written communication skills.
Experience and education in leading support groups.
Demonstrated understanding of hospice philosophy and principles.
Demonstrated understanding of the principles of grief and loss and the skills needed to assist the bereaved in the healing process.
Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation, ability to work effectively within an interdisciplinary team, and ability to demonstrate appropriate independent judgment.

Summary:
The Hospice Aide is a vital member of the interdisciplinary team. He/she is assigned to care for patients who require personal care services. The Hospice Aide reports to the Clinical Manager or Hospice House Manager, but receives direct supervision from the RN Case Manager or designee.
The Hospice Aide is responsible for providing and/or assisting the patient with personal care needs, ambulation, supplementary services including meal preparation, and other appropriate tasks as indicated by the RN Case Manager or designee. Charting on the appropriate agency forms is also required.
Qualifications
Qualifications:
• High School Diploma or equivalent preferred.
• Certification of successful completion of a CNA or CNA/ Home Health Aide course approved by the Massachusetts Department of Public Health and in accordance with the requirements of the U.S. Department of Health and Human Services.
• Certification must be in good standing with the nurse aide registry and not have had a lapse in furnishing services within a 24 month period.
• In lieu of an HHA certificate, successful passing of hospice aide written test and skills assessment.
• Experience in the field as a home health aide or certified nursing assistant is preferred.
• Availability to work day, evening or night hours, including weekends and holidays as required.
• Has a compassionate, warm and accepting attitude toward people.
• Demonstrates a sympathetic, mature attitude in caring for the sick and disabled and an ability to deal with different ideas and value systems.
• Ability to respect confidentiality of patients and families.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.

Position Summary:
We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW or LICSW) to join our hospice care team, dedicated exclusively to weekend coverage. This role is critical in supporting patients and families through end-of-life care by providing psychosocial assessments and crisis interventions. The Hospice Social Worker will work closely with the interdisciplinary team to deliver timely, patient-centered care and will be responsible for weekend patient visits, including Service Intensity Add-On (SIA) visits for those nearing end of life, as well as admission inquiry visits.
Key Responsibilities:
Perform weekend patient visits, ensuring continuity and quality of care.
Respond to and complete Service Intensity Add-On (SIA) visits for patients in the last days of life, providing supportive interventions during imminent death.
Conduct admission inquiry visits, assessing patients for hospice eligibility and collaborating with the clinical and admissions team as needed.
Conduct psychosocial assessments and develop individualized care plans for patients and families receiving hospice care.
Offer counseling, emotional support, and anticipatory grief support to patients, caregivers, and families.
Participate in weekend interdisciplinary communication to ensure coordinated care planning and updates.
Document all assessments, interventions, and outcomes in accordance with agency and regulatory standards.
Maintain compliance with Medicare hospice regulations, ethical standards, and agency policies.
Provide crisis intervention and assist with emergency psychosocial situations that may arise over the weekend.
Maintain regular communication with the Director of Continuity of Care and contribute to ongoing quality improvement initiatives.
Qualifications:
Master's degree in Social Work (MSW) from an accredited institution.
Current and active LCSW or LICSW license in Massachusetts.
Minimum of 2 years' experience in hospice, palliative care, or a related clinical setting strongly preferred.
Demonstrated knowledge of end-of-life issues, bereavement, and grief counseling.
Exceptional interpersonal, communication, and organizational skills.
Ability to work independently and manage time effectively during weekend shifts.
Reliable transportation and valid driver's license (if required for home visits).
Work Environment & Schedule:
This is a weekend-only position; shifts typically occur on Saturday and Sunday, with flexibility depending on patient needs and acuity.
Work may be performed in patient homes, facilities (e.g., nursing homes, assisted living), or inpatient hospice settings.

Job Description
Summary:
The Hospice Aide is a vital member of the interdisciplinary team. He/she is assigned to care for patients who require personal care services. The Hospice Aide reports to the Clinical Manager or Hospice House Manager, but receives direct supervision from the RN Case Manager or designee.
The Hospice Aide is responsible for providing and/or assisting the patient with personal care needs, ambulation, supplementary services including meal preparation, and other appropriate tasks as indicated by the RN Case Manager or designee. Charting on the appropriate agency forms is also required.
Qualifications
Qualifications:
• High School Diploma or equivalent preferred.
• Certification of successful completion of a CNA or CNA/ Home Health Aide course approved by the Massachusetts Department of Public Health and in accordance with the requirements of the U.S. Department of Health and Human Services.
• Certification must be in good standing with the nurse aide registry and not have had a lapse in furnishing services within a 24 month period.
• In lieu of an HHA certificate, successful passing of hospice aide written test and skills assessment.
• Experience in the field as a home health aide or certified nursing assistant is preferred.
• Availability to work day, evening or night hours, including weekends and holidays as required.
• Has a compassionate, warm and accepting attitude toward people.
• Demonstrates a sympathetic, mature attitude in caring for the sick and disabled and an ability to deal with different ideas and value systems.
• Ability to respect confidentiality of patients and families.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.
