The Providence Center CaseManager ACT II MHPRR Outpatient Team provides community-based casemanagement and supportive services within TPC's Certified Community Behavioral Health Clinic (CCBHC) model. Individuals served on the ACT II team have complex Serious and Persistent Mental Illness (SPMI), reside in an MHPRR (Mental Health Psychiatric Rehabilitative Residence) and benefit from a coordinated, team-based approach to care. This level of service is designed for individuals who may not require the ACT I (High Intensity) level of support but would benefit from multidisciplinary collaboration to assist with treatment, rehabilitation, and community integration. The CaseManager works closely with clients to promote recovery, stability, and independence through consistent engagement, care coordination, and linkage to community resources. The primary work in this role is to provide CCBHC services to the MHPRR population, in their residential environment.
Duties and Responsibilities:
Deliver person-centered, recovery-oriented casemanagement services as part of a multidisciplinary team providing comprehensive treatment, support, and rehabilitation to individuals with Serious and Persistent Mental Illness (SPMI).
Ensure service intensity aligns with ACT II (Moderate Intensity) standards, including a minimum of five (5) or more client contacts per month, with at least three (3) contacts conducted in person. These contacts may be provided by any member of the multidisciplinary team to meet client needs in their residential environment.

Duties and Responsibilities:
Provide casemanagement and support to consumers.

-Involve consumer, agencies and others in treatment planning.
-Collaborate with clinical team on engagement insight and needed supports.
-Provide support counseling with mental health issues, substance use disorders, and medical conditions.

-Assist and monitor the group home consumers with medical interventions and hygiene needs.

-Teach, supervise and assist consumers with doing household chores (morning and evening, laundry, room cleaning, etc.) using realistic situations.
-Teach clients coping skills to implement symptoms management and harm reduction.
-Provide direct alcohol and drug counseling/casemanagement to individuals with substance use disorders.
-Perform household duties as needed.
-Use a step-by-step process when teaching skills.
-Use positive reinforcement and encouragement.
-Integrate skills training in all aspects of their treatment plan.
-Maintain a clean, healthy and safe environment at all times.
-Adhere to Infection Control protocols.
-Implement emergency protocols when required.
Initiate treatment and advocate for consumer within network of care.
-Negotiate and schedule appointments for consumers within The Center and with outside agencies (Social Security Supplemental Insurance (S.S.I.), Food Stamps, Advocates Office, Medical Doctor, etc.).
-Negotiate on behalf of the consumer, as needed, with landlords, courts, families, treatment team, consumers themselves.
Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
-Develop professional relationship with Day Program staff.
-Provide and maintain a home atmosphere.
-Maintain and promote family contacts and relationships.
Transport consumers in own vehicle or Center van to planned activities and appointments.
-Drive carefully and abide by traffic rules.
-Ensure that seat belts are worn by all traveling in Center or owned vehicles.

-Complete required interagency and other necessary paperwork.
-Complete shift activity logs.

-Complete required reports and evaluations (ADL/DLA Assessments and Psycho-Social Functional Assessments).
-Follow required office procedures in a cooperative manner.
-Initiate and process incident reports as per Center guidelines.
Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
-Communicate with various local agencies and other departments or teams at The Center.
-Provide appropriate referrals and supports to identify and target co-occurring needs.
