The Providence Center Case Manager ACT II MHPRR Outpatient Team provides community-based case management and supportive services within TPC's Certified Community Behavioral Health Clinic (CCBHC) model. Individuals served on the ACT II team have complex Serious and Persistent Mental Illness (SPMI), reside in an MHPRR (Mental Health Psychiatric Rehabilitative Residence) and benefit from a coordinated, team-based approach to care. This level of service is designed for individuals who may not require the ACT I (High Intensity) level of support but would benefit from multidisciplinary collaboration to assist with treatment, rehabilitation, and community integration. The Case Manager works closely with clients to promote recovery, stability, and independence through consistent engagement, care coordination, and linkage to community resources. The primary work in this role is to provide CCBHC services to the MHPRR population, in their residential environment. Working with consumers to achieve the highest level of independence possible. Duties and Responsibilities: Initiate treatment and advocate for consumer within network of care. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy. Provide case management and support to consumers. Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment. Teach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments. Initiate treatment and advocate for consumer within network of care. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. Transport consumers in own vehicle or TPC van (if available) to planned activities and appointments. Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy. Maintain cooperative relationships with Center staff, consumers, community agencies and the public. Other duties as assigned. Requirements: High School Diploma/GED required; Associate's or Bachelor's degree preferred. If no higher education degree, must have 5 years of relevant behavioral health experience. Experience in residential facilities is preferred. Bilingual Spanish preferred.
