Represent the Community Diversion program in both internal and external meetings for the organization. Organize and support the teams daily delivery of service. Meet program goals/standards of care in accordance with Center policies and BHDDH regulations. The Healthy Transitions Nurse must have professional knowledge and understanding of the characteristics of children and young adults with mental health issues, their families, and family systems. Knowledge of community resources or ability to learn quickly as well as good organizational, time management and communication skills. Ability to work well with community partners and in a team environment is essential. Highly motivated, self-directed, willing to work a variety of hours, including evening and on-call.
