Communicate with physicians and other involved disciplines in patient care. Evaluate referral orders to determine adequacy of nutrition intake methods. Qualifications * High school diploma or equivalent. * Current state certification (or registration) as a nurse assistant as per state certification/registration requirements. * CNA experience within the last two years. * Current CPR certification. * Valid driver's license, reliable transportation and liability insurance.
