The Afterhours Registered Nurse's Primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on the weekends.
Able to triage patient needs over the phone until a nurse can provide an in-person visit.
Responds appropriately and with urgency to crisis calls from patients and families afterhours.
Makes the initial nursing evaluation in determining eligibility for hospice services, as part of a patient visit, within forty-eight (48) hours of referral if assigned.
Competent to perform afterhours visits including admission visits, death visits, recertification visits, and routine visits.
Identifies the patient/family's physical, psychosocial, emotional, and environmental needs and reassess them as needed, no less than every fifteen (15) days.
Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
Experience Requirements
One year of clinical experience.
License Requirements
Current RN licensure in the state of practice and one year of clinical experience.
Current CPR Certification.
Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Essential Responsibilities:
Directs/manages hospital operations on assigned shift to ensure that appropriate safe patient care is provided and patient/family needs are met.
Rounds on all patient units to identify patient placement needs, available beds with assigned staff and staff capacity to safely take additional admissions, at least once every three (3) to four (4) hours. Communicates at Huddles and directly with Charge Nurse to: Review current patients. Identify pending/potential discharges. Determine capacity for and time frame for admissions. Discuss any anticipated internal transfers in department to facilitate patient care needs or room for admission. Support regional utilization strategy.
Works with Unit Managers and their designee, EPro (patient admission transfer center) Surgical Services, Sterile Processing, Inpatient Med/Surg, the Advanced Urgent Care/Emergency Department, Care Coordinators, outside facilities, Physicians/APPs, and KP Clinics, to assure appropriate prioritization and placement of patients, and Food & Nutrition Services Staff.
Coordinates with Managers and/or their designee to ensure appropriate staff is provided on each shift to meet patient needs and maintain cost effectiveness. Collaborates with charge nurses/managers/directors to initiate additional resources for planned or unplanned events.
Ensures compliance with organizational policy and procedures and with regulatory requirements.
Anticipates potential problems and intervenes to minimize adverse impact; manages conflicts that arise. Assists with decedent affairs including organ/tissue procurement procedures.
Acts as a clinical resource for nursing including interpretation of policies and procedures and assisting with clinical practice issues. Supports performance improvement activities.
Assists departmental staff in responding to and resolving patient/family questions, concerns or complaints when management team not available.
Assists with the on-the-job injury management program.
Responds to emergency codes (Rapid Response, Code Blue and other facility/patient emergency calls) to ensure appropriate support is available.
Appropriately communicates and documents administrative, departmental, risk management and member issues to ensure timely follow up and response.
Coordinates with Engineering regarding equipment malfunctions, safety issues and repair needs, and ongoing patient room maintenance.
Initiates Disaster Procedures as necessary and functions as incident commander until relieved.
In absence of Hospital Department Managers, addresses and communicates employee issues that arise after hours. Communicates issue and problem resolution actions to department manager.
Basic Qualifications: Experience
Minimum three (3) years of acute care nursing practice.
Minimum one (1) year of leadership experience to include Charge/Lead RN in health care field.

The OB Hospitalists support the inpatient needs of the Legacy Medical Group (LMG) Maternal Fetal Medicine service. The OB Hospitalists and the MFM providers work closely together on a daily basis, and the MFM practice has 24/7 (not in-house) coverage available. The OB Hospitalist also provides backup to the LMG Midwifery Practice (all CNMs), coverage for unassigned labor/triage patients, support of our private practice OB colleagues as needed (i.e. C-section assisting). Both locations have Level IV NICUs and serve a vital role in the surrounding communities for high-risk pregnancies. Dedicated in-house Anesthesia is present 24/7 at both sites. Local academic OB/GYN and Family Medicine residents provide coverage 24/7 at LEMC, with occasional medical student rotations at both sites. No clinic responsibilities.

Collaborates with other nursing leaders to implement strategies to monitor and evaluate nursing orientation programs, continuing education and skills development programs.

Responsibilities:
IMAGING
Provides quality images with accuracy and efficiency, using optimal skills and knowledge of all equipment.
Competency in the field of MRI.
Positions patient for diagnostic imaging according to department procedures, training, and machine requirements.
Operates imaging equipment, determines and sets exposure factors, sets technique utilizing all relevant factors to ensure accurate and quality images with minimal and acceptable repeat rates.
Uses computer image processing to select and optimize images.
Processes and presents images and related patient information to radiologist for diagnosis.
Assists with patient transport by moving patients using standard procedures.
DOCUMENTATION
Gathers complete documentation of all pertinent information to provide accurate and timely service to customers.
Identifies patient by checking patient's I.D., full name and medical record number according to department procedure.
Reviews patient chart and orders to insure correct examination.
Labels images completely with demographic, position and other needed documentation.
Logs activities, gathers charging information and charges exams as required.
Follows department and hospital procedures and policies.
Performs quality control and quality assurance, records results and reports variance from normal equipment function.
Performs medication reconciliation on outpatients receiving IV contrast media.

Experience:
One year previous experience in the field of MRI preferred. Keyboard skills and ability to navigate electronic systems applicable to job functions.
Licensure:
Licensure/Certification:
ARRT certification in Radiography, or ARRT certification in MRI required.
If not MRI-certified at time of hire, ARRT certification in MRI, or ARMRIT certification in MRI required within six months.
Current state license in Magnetic Resonance Imaging required for all employees who perform this job in the state of Oregon.
American Heart Association BLS/CPR Certification: Personnel providing direct care in patient care and diagnostic areas are required to acquire and maintain BLS/CPR certification.
