The Hospice Home Health Aide is a certified nursing assistant. Under the guidance and supervision of a registered nurse or therapist, provides personal care and related services for the hospice patient in order to augment care provided by the family and other caregivers and to maintain a clean and safe environment. Performs homemaking tasks as assigned. Assists with treatments as ordered by the physician and instructed and supervised by the nurse, such as change of a colostomy bag, shampoo, simple wound care, reinforcement of a dressing, simple urine tests for sugar, acetone, and albumin, and measuring intake and output. Encourages patient's mental alertness through involvement in activities of interest. Gives simple emotional and psychological support to the patient and other members of the household or facility. Encourages the patient to become as independent as possible according to the plan of care. Assists patients with bathing and personal care as directed by the plan of care. Monitors vital signs as directed by the plan of care. Monitors skin integrity and provide skin care as directed by the plan of care.
