Works cooperatively with other agencies that provide parent education. Takes training online and on site to better educate parents and as required for continued training hours.
Prepares and submits periodic reports on program efforts and needs to the Ohio Department of Children and Youth, Family and Children First Council, the Early Childhood Collaborative and others as needed. Monitors and enters OCHIDS data, monitors referrals to ensure accuracy and proper utilization.
Assists in activities to create public awareness of the Help Me Grow Program.
Completes office work such as documentation and data entry, scheduling visits, making phone calls and preparing letters to clients or referrals, receives and responds to inquiries; etc.
Attends program staff meetings; participates in regularly scheduled supervision and attends HCHD staff meetings and trainings.
Maintains the required licensure and certification after gaining certification with the support of the HMG supervisor.
Demonstrates regular and predictable attendance and meets all job safety requirements and all applicable OSHA safety standards that pertain to essential functions.
Performs all other related duties as assigned or directed to promote, further, and ensure the effective and efficient operation of the Holmes County General Health District.

The Care Companion supports our mission and values by performing various activities to meet the care and/or service needs of a client in their home environment, including personal care assistance, meal preparation, housekeeping, laundry, transportation, activities, and resident support services as well as participate in improving quality of care/services.
The Care Companion's primary responsibilities include:
Prepares specific food items following appropriate standards for food preparation, handling, and services.
Serves client meals ensuring the temperature, portions, and timeliness of the service.
Performs a variety of general cleaning duties for the entire house which includes but is not limited to: dusting, sweeping, mopping, vacuuming, cleaning windows, cleaning carpets.
Perform laundry duties such as washing, drying, folding and putting away
Empties waste containers, collects and disposes of recyclables and garbage.
Change bed linens and make beds.
Collects soiled laundry: washes, dries, folds, irons, performs minor clothing repairs and distributes laundry.
Delivers mail and packages to client.
Transport client when needed.
Assists with all client needs for personal care services.
Inform supervisor of client needs and concerns.
Attends and participates in Brethren Care At Home team meetings as required.
Attends in-services outside of Brethren Care Village and/or Brethren Care At Home for support and needs of the client
Other duties as assigned.
What do I need for this role?
High School Diploma or equivalent preferred but not required.
Training in geriatrics (nursing, social work, rehabilitation or therapeutic recreation) or sufficient related prior work experience is preferred but not required.
Onsite presence required.

The primary purpose of your job position is to care for residents with impaired ability to perform at an independent level due to disease process, under the supervision of a licensed nurse.

Ability to give AM and PM care. Perform ADL's. Give showers, baths and whirlpool. Ability to lift, turn and transfer residents. Ability to use the Hoyer Life per facility policy
Ability to serve breakfast, lunch and dinner trays and assist the resident as needed
Ability to change incontinent residents
Ability to walk ambulatory residents
Ability to complete tub baths, showers, bed baths and whirlpools
Ability to report anything unusual about residents to Charge Nurse
Ability to empty and record all urinary drainage units
Ability to practice Infection Control procedures. Ability to report any Bloodborne Pathogen injuries top your supervisor immediately. Have knowledge of the location of the eye wash system
Ability to follow Resident's Rights policies at all times
Ability to consistently work cooperatively with residents, charge nurses, treatment nurses, restorative assistance, other nursing assistance, physicians, families, consultant personnel, and ancillary service providers.
Ability to observe all facility safety policies and procedures
Ability to come to work in a clean, neat uniform, and consistency present a professional appearance
Ability to come to work as scheduled and consistently demonstrates dependability and punctuality
Ability to accept assigned duties in a cooperative manner
Ability to assume accountability for data contained in the employee's handbook
