The Direct Service Provider (DSP) delivers basic care to developmentally disabled adults in the individual's own private residence. Care is provided in accordance with the Personal Plan of Service which is different for each person being served depending on their own unique needs. These needs could include but are not limited to personal care, transportation, teaching, guiding, and supporting individuals in meeting their own personal needs, dreams, and desires. RESPONSIBILITIES: Perform all assignments in accordance with established organizational policies and procedures as outlined in the organizations' policy manual, employee handbook, and as instructed by the supervisor. Provide direct care according to the individual's plan of care (ISP). Documenting care and medication according to company and state policy and procedures. Input clinical data into Home Health Software as instructed. Meet with the family to discuss the needs of patients. Teach developmentally disabled adults how to adopt basic independent living skills. Help physically disabled move from room to room around their homes. Assist individuals in dressing, bathing, and completing basic hygiene tasks. Oversee any required rehabilitation exercises. Ensure that individuals take their prescribed medications or administer medications: paid training provided. Prepare meals that adhere to special dietary considerations.
