This position provides primary care to individuals of all ages and backgrounds within our Federally Qualified Health Center. Our clinics offer comprehensive acute and chronic care, with an emphasis on equitable, trauma-informed, and culturally responsive services.
The provider will deliver patient-centered care that includes diagnosis, treatment, and management of acute and chronic conditions, preventive health services, and health education. Working collaboratively within a multidisciplinary care team, the provider will promote continuity of care through joint planning and problem solving with internal and external partners.
Advanced Practice Provider Starting Salary Range: $131,421.66 to $145,064.93 per year
Physician Starting Salary Range: $225,860.02 to $249,307.20 per year
Advanced Practice Provider Essential Duties
* Provide patient-centered care in a certified medical home model.
* Provide a wide range of health care services including the diagnosis and management of acute, chronic, and complex health problems, health promotion, disease prevention, health education, and counseling to individuals, families, groups and communities.
* Perform history taking, physical examinations, diagnosis and therapeutic treatment planning consistent with evidence based practice guidelines, clinical policy and procedures, and the scope of practice.
* Perform minor procedures, biopsies, and pelvic exams.
* Order and interpret diagnostic lab and radiologic tests assuring that results are communicated to individual patients or a parent/guardian of the patient.
* Share after-hours call with other staff providers.
* Meet all regulatory obligations of the Health Center's medical practice: FTCA, CLIA, HIPAA, State and Federal regulations and other appropriate regulatory bodies as applicable.
* Fully participate in the clinic and medical staff development through involvement in service evaluations, health care plans, chart reviews, and program development.
* Act as a resource to the care team and communicate appropriate knowledge, skills, and conduct.
* Actively participate in quality improvement initiatives aimed at improving population health outcomes.
* Maintain a record of assessments, plans, treatments and referrals.
* Support the organization as needed.

Physician Essential Duties
* Provide outstanding acute and chronic care to low income children and adults who have limited resources
* Work in collaboration with multidisciplinary provider team (allopathic, naturopathic, acupuncturist, dentists) at OI to provide innovative and evidence-based care
* Maintain specialized clinical skills as applicable, e.g., minor procedures and biopsies, pelvic exams, blood draws
* Provide medications management for low acuity mental health patients in conjunction with behavioral health and psychiatric consultation as appropriate for a primary care setting
* Maintain a panel of Suboxone opiate replacement therapy patients in conjunction with the agency's alcohol and drug treatment case management program (may obtain waiver after employed)
* Share after-hours call with other staff practitioners
* Meet all clinical obligations of the Health Center's medical practice: CLIA medical, HIPAA, State and Federal regulations, etc.
* Participate in the Health Center's Quality Improvement and care delivery changes in coordination with regional health reform initiatives
* Fully participate in the Clinic and medical staff development through involvement in service evaluations, health care plans, chart reviews, and program development

This individual will work collaboratively within an interdisciplinary team to provide Person-Centered mental health treatment services in a 24-hour residential setting, serving adults with serious and persistent mental illness or chemical dependency. This position is an opportunity to build experience working in a therapeutic milieu environment, administering medications, maintaining documentation and communication with staff, and transporting residents as needed - ODL is required. This is a potential opportunity to gain hours for credentialling. Temp/On-call positions generally help cover 8-10-hour shifts as needed, the most emergent need is graveyard coverage. Temp/On-call Aides are required to cover at least two shifts per month and may take on as many additional shifts as they are willing/able.
GENERAL STATEMENT OF DUTIES
Assists in assessing health and social service needs of a specific population through home visits and visits to other agencies and social services resources; refers clients to appropriate resource; advocates to obtain required services as necessary; provides information and education on health-related issues.

POSITION SUMMARY

KEY RESPONSIBILITIES
Assist residents with activities of daily living, such as bathing, dressing, grooming, toileting, and transferring, as needed, while adhering to community protocols, licensing regulations, and guidelines for resident and employee safety.
Prioritize independence and emotional support for the residents by providing compassionate care, engagement, and companionship.
Encourage and assist with participation in life enrichment activity programs.
Assist with light housekeeping duties, such as making beds, tidying rooms, etc.
Promote open communication between healthcare professionals, families, residents, and staff.
Requirements:
CANDIDATE QUALIFICATIONS
Education:
High School Diploma or equivalent.
Current caregiver training/certification per state requirements, such as CNA/GNA/PCA/DCA, or the willingness/ability to obtain.

• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Report any known or suspected unauthorized attempt to access facility's information system.
• Perform only those care procedures that you have been trained to do.

Job Description
Schedule: Varying Days (Full Time 32-40hrs/week)
Hours: Varying Day or Swing Shift
Job Summary: The identified duties and responsibilities are representative of the basic functions, general nature and level of work for the position. They are not to be construed as an exhaustive list of the duties required. Every Team Member is expected to perform essential functions that are similar or related to the ones identified, and to follow supervisor direction and complete additional duties as requested and assigned. The Caregiver is responsible for assisting residents in their Daily Living Activities including, but not limited to: bathing, grooming, personal and dental hygiene, transportation to meals and activities, bowel and bladder functions, etc., maintaining resident dignity and respect with superior customer service and in accordance with all policies, procedures, regulations and guidelines.
Essential Job Duties (Include % of time for each responsibility):
- Respond to resident needs promptly and kindly while maintaining each resident's self- respect, personal dignity, personal safety and confidentiality (50%)
- Read and follow care plans to ensure proper care is being given to their residents (10%)
- Assist residents in their daily living activities including bathing, dressing, hygiene (including shaving, hair care, etc.), meals, activities, bowel and bladder functions (including taking to bathrooms, portable commodes, changing incontinency products, etc.)(20%)
- Provide assistance at mealtime with selecting meals, serving, eating, clearing, delivering room trays as needed or assigned (3%)
- Transport residents as necessary assisting them into and out of bed, wheelchairs, etc. following all safety/lifting and transferring techniques - escorting them to/from meals, activities, or other transportation needs as requested or assigned (3%)
- Perform regular safety checks on residents (1%)
- Encourage residents to participate in daily activities such as bed making, laundry, cleaning, setting tables, etc. (1%)
- Invite, encourage, assist and lead residents in meaningful and life enriching activities (3%)
- Clean, complete laundry, and assist with all other community activities and job duties as required or assigned(1%)
- Manage oxygen tanks and concentrators per MD orders for resident needs (1%)
- Chart and document appropriate information (including incident reports) communicating any observed or suspected resident change of condition to a supervisor immediately (4%)
- Respond promptly to all resident alerts, carrying walkie- talkies and reacting to emergency or other resident requests as needed (3%)
- Ensure resident safety and comfort at all times, assisting in the performance of fire or other safety drills (1%)
Non- Essential Job Duties:
- Always display a positive and professional image through actions and dress and in accordance with the MBK core values
- Encourage teamwork through open communication with co- workers and other departments
- Demonstrate effective time management skills so that all resident assignments are completed daily per shift
- Display tact and friendliness when dealing with residents, families, and guests
- Adhere to all MBK policies and procedures related to meal breaks and rest periods
- Perform other duties as consistent with the position and as assigned by the Director of Health Services, and/or Director of Assisted Living, and/or Director of Memory Care, and/or Executive Director
Requirements (Include education, experience, special skills, licenses, certifications):
- Age 18 or over, or have the proper work permit and work authorization documentation
- High school diploma or equivalent
- Nurses Aid certification preferred
- Current first aid certification
- Background clearances as required by government regulations
- Complete a health screening and provide negative TB test results (must be within the last 6 months or within 7 days of employment)
- Prior Caregiving experience preferred but on the job training is available for candidates with the right attitude and desire
- Must be able to read and speak English
- Ability to follow oral and written directions
- Ability to interact effectively with people of different ages and backgrounds
- Must meet and maintain all state specific regulatory requirements
Physical Demands (Include lifting, carrying, pushing, pulling, bending, walking, standing, sitting):
- Must be mobile and able to execute the physical demands of the Caregiving position including kneeling, squatting, overhead reaching, bending
- Must be able to lift 40+ pounds and push 150 pounds unassisted
