Record and report data as instructed • Perform special treatments as instructed. • Immediately notify the Nurse of any resident leaving/missing from the facility. • Use only the equipment you have been trained to use. • Report defective equipment to the Nurse Supervisor/Charge Nurse • Maintain the confidentiality of all resident care information including protected health information. • Report known or suspected incidents of unauthorized disclosure of such information. • Report all allegations of resident abuse and/or misappropriation of resident property. • Honor the resident's refusal of treatment request. Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols. Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care. Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes. Present for staff meetings and training. Other duties as assigned. REQUIREMENTS: Must be 18 years of age High school diploma or equivalent required. Previous experience in caregiving, home care, or community-based care is preferred. Ability to work independently and as part of a team. Patience, empathy, and a genuine desire to help others. Good organizational and time management skills. Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility. Must be able to stand, walk, and reach for extended periods. Ability to pass a background check.
