The Caregiver will provide assistance with daily living activities for residents with Alzheimer's/Dementia. You may be required to respond to emergency calls and situations, document and report care provided, and transport residents to various functions or locations. Familiarity with Alzheimer's/Dementia, and a willingness to work with the elderly population is essential for this role.

The Caregiver will:
Assist with activities of daily living such as grooming, oral hygiene, toileting, bathing/showering, eating, etc.
Assist with personal and household chores, such as tidying up room, errands, delivering mail and/or newspapers.
Transport residents within the community to meals, enrichment activities, and other programs as needed.
Ensure resident care plans are reviewed and followed consistently.
Observe and monitor residents for behavioral changes and/or adverse reactions to medications or treatment plans. Communicate any changes to clinical staff immediately, as well as document changes thoroughly.
Respond to emergency calls, resident/visitor's requests and correct environment hazards in a timely, appropriate and safe manner.
Observe community policies and procedures regarding resident and team member safety, attendance, timeliness, house rules, teamwork, and resident service.
Maintain professionalism and resident confidentiality at all times.
Adhere to all Infection Prevention, OSHA, fire and safety regulations at all times, to ensure the safety and well-being of all community residents.
Perform all other duties as requested.
Care Partner Educational Requirements:
Must possess a high school diploma or equivalent.
Caregiver, CNA or Home Health Aide certification is preferred, but not required.
Fulfill and maintain continuing education credits as required by state.
Must be willing to obtain and/or maintain current First Aid/CPR certification.

As a Direct Support Professional (DSP) / Caregiver, you begin each shift ready to help adults with developmental disabilities live their best lives as independently as possible. You have made it your mission to ensure the comfort and safety of each client. In our residential group home, you assist our residents with activities of daily living such as administering medication, cooking meals, performing personal hygiene, and transportation to medical appointments.
You also assist in the implementation of individual behavioral plans. Using effective teaching and mentoring techniques, you teach them new skills and help them achieve goals. Based on each individual's preferences and behavioral plans, you encourage their interests and drive them to community outings and recreational activities. You are happy to help them find greater access to the community. You are conscientious about reporting any concerns about their health and safety and keeping documentation accurate and up-to-date.
Some aspects of this job are not easily accomplished but at the end of each shift, knowing your clients are happy makes you feel great about the work you do!
WORK SCHEDULE
We operate 24-hour residential group homes which means our Direct Support Professionals (DSPs) / Caregivers have many shift options including mornings, afternoons, evenings, overnights, and weekends.

Alvita Care is a mission-driven Home Healthcare & Personal Care Agency with four branches and over 500 caregivers. We provide relief and assurance to family members in the areas of In-Home Personal Care, Alzheimer's & Dementia Care, Palliative & Hospice Care, and Geriatric Care Management and have earned The Joint Commission Gold Seal of Approval. Alvita Care clients are all private-pay and we do not accept Medicare or Medicaid.
As we expand our circle of care, we are seeking top-notch caregivers to service our Connecticut needs across the five boroughs. We have private-duty one-on-one care cases available as well as facility-based care cases.
All applicants are subject to a phone screening to determine compatibility and overall availability of accommodations for both the caregiver and the agency. Please note that cases with Alvita Care are only available to caregivers who successfully complete the onboarding process.
QUALIFICATIONS:
Valid HHA/PCA certification in the state of NY.
2+ years of professional home care or healthcare experience
Reliable, punctual, and professional demeanor
Completed drug screen or completed Habituation Form
Must successfully complete Alvita Care process to accept cases
Ability to remain calm under pressure and handle challenging behaviors with care
Trustworthy, reliable, and compassionate
Memory Care, Palliative, ADL, and transfer experience are all a plus
DETAILS:
Full-time, part-time, & per diem schedule options are available
6 - 12 hour day time shifts available; overnights available occasionally
Weekend availability and flexibility with shifts
