Client is looking for skilled Nurse Practitioners and Physician Assistants eager for the opportunity to make a difference in patients' lives. The Advanced Practitioner at Client is a key member of an integrated Care Team, which includes a Registered Nurse and a Social Worker. The patients we serve often struggle with multiple serious diseases. Our Nurse Practitioners and Physician Assistants help patients improve their quality of life in the home and slow the progression of kidney disease, enabling positive health outcomes.
Using your skills in this position will allow you to deliver personalized, compassionate medical care to individuals mainly chronic illness or kidney disease. You will also be responsible for caring for patients, maintaining accurate and current patient records and scheduling, and administering follow-up appointments to patients as required. Your gifts as a healthcare professional are urgently needed. In healthcare systems, the patient has too often become secondary due to processes and incentives that don't positively impact the patient for the long term. We strive to change that narrative by putting our patients and their quality of life at the forefront of what we do.
Roles and Responsibilities
• Conducts assessments, including comprehensive annual wellness exams on patients both in the patients' home and in the virtual environment
• Counsels and educates patients and families about benefits and programs available to help them live healthier lives
• Documents items such as: appropriate chief complaint, all applicable diagnoses, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan
• Responsible for the coordination of care with primary care providers, specialists, and appropriate ancillary services
• Completes all documentation and paperwork in a timely manner
• Maintains quality of care standards as defined by the practice
• This position will not be office-based but will be remote in the state in which employed and will need to attend periodic training/meetings outside of that state
• Delivers evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first
• Prescribes medications, orders tests, and collaborates with the patient's Client physician
• Performs effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily
• Facilitates closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing
• Assists patients with enrolling to access educational videos
• Participates in the integrated care team meetings
• Knowledge of disease diagnosis and prevention
• Makes assessments of patients' health status
• Develops treatment plans
• Implements plans consistent with appropriate plans of care
• Follows up and evaluates patients' status
• Other duties as assigned
Position Requirements
• Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
• Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
• Current and unrestricted DEA certificate
• Ability to work without direct supervision and practice autonomously
• Access to transportation, a valid driver's license, and car insurance
• Must be proficient with medical instruments and equipment required by the work
• Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology
• Ability to communicate effectively in verbal and written form with retail and medical partners at various levels, patients, family members, physicians, and representatives of the community
• Sound understanding of all federal and state regulations including HIPAA and OSHA
• 2 or more years of direct patient care required
• Managed Care/IPA/Health Plan experience
• Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home

In the CTS Fellowship, we will immerse the Fellow in all aspects of Cardiothoracic Surgery from preoperative care to postoperative critical care and post-discharge care. In the CTS Fellowship, APPs are giving the unique opportunity to provide perioperative care for CTS patients from initial consultation to discharge. The CTS Fellow will spend the majority of their Fellowship in the Cardiac Surgery Intensive Care Unit (CSICU). In collaboration with established APPs, Fellows will have the opportunity to master their skills including:
Critical Care Management: Fellows will manage patients recovering from various cardiothoracic surgeries, including aortic surgery, coronary artery bypass grafting, thoracic surgery, and valve surgery.
Collaboration on Mechanical Circulatory Support: Fellows will work closely with APPs and attending physicians to manage complex mechanical circulatory support devices such as ECMO, Intra-Aortic Balloon Pump (IABP), Left Ventricular Assist Devices (LVADs), Impella, and Right Ventricular Assist Devices (RVADs).
Multidisciplinary Communication: Fellows will master the essentials of communicating the plan of care effectively with the multidisciplinary team, especially regarding any changes in the patient's condition.
Procedural Skills: Fellows will have opportunities to enhance their procedural skills in common intensive care settings, including bronchoscopy, central line placement, arterial line insertion, and chest tube management.
Patient Experience Optimization: The Fellows role will also focus on optimizing the patient experience, satisfaction, quality of care, and overall outcomes through effective perioperative care.
Diagnostic Test Management: In collaboration with the APP team, Fellows will be responsible for ordering and interpreting a range of diagnostic tests pertinent to each patient's condition.
Emergency Response: In critical situations, Fellows will respond to emergencies, providing medical stabilization until definitive management can be initiated.

POSITION SUMMARY
A Personal Care Assistant is a Paraprofessional whose role is to increase the quality of education by assisting one particular student with instructional and non-instructional activities. Personal Care Assistants (PCAs) are not responsible for the design, direction or evaluation of the teaching/learning process. May be used in any instructional situation under the direct, immediate supervision of a certified professional to perform those tasks specified in the . The role of the assistant varies according to the program assignment.
ESSENTIAL FUNCTIONS
Assists the assigned student on and off the bus at arrival and departure times.
Know the whereabouts of the student assigned to you at all times. Report missing children to the office immediately.
Assists and/or monitors the assigned student with the removal of putting on/off wraps.
Facilitates personal care services as needed to assist the student with daily living activities necessary for comfort and well-being (when indicated on Student's IEP).
Performs duties such as locking and unlocking braces in order to facilitate changes in position; assist children in the bathroom and assists with lifting non-ambulatory students, as necessary. Assists, in cases of emergencies, where the nurse may not be immediately available, all possible measures should be taken to assist the child until help arrives. This includes notifying the principal or other building administrator immediately.
Assists with the supervision of the assigned student during play, recess, and recreational activities.
Assists with feeding the assigned student if he/she is unable to feed him/herself. In addition, may collect lunch money, submit lunch count information and supervise students in the Cafeteria as needed.
Performs reinforcement instructional services as delegated by the teacher, including reinforcing activities in the visual, auditory, motor and cognitive areas.
Attends training as required for performance of job. This may include, but is not limited to, CPI, CPR, First Aid, Dysphasia, and Universal Precautions.
Accompanies the teacher and assigned student on field trips or community based instruction during the school day.
Follows and reinforces the student's behavior plan as instructed by the classroom teacher.
Accompanies the assigned student and assists the teacher during emergency drills.
