Responsibilities:
Care for residents and assist in updating each resident's service plan

Position Summary
The Mobile Integrated Healthcare (MIH) Paramedic works as part of a multidisciplinary team to provide proactive, post-hospital, and community-based care in non-traditional settings. This role focuses on continuity of care through in-home visits, patient assessments, clinical and social screenings, patient education, and coordination with healthcare and community partners. The MIH Paramedic functions independently while collaborating closely with physicians, nurses, behavioral health providers, and community agencies to improve patient outcomes and reduce unnecessary emergency department utilization.
Work Environment & Schedule
Non-traditional care settings including patient homes and community locations
Shift work includes seven 12-hour shifts in a two-week period
Required Qualifications
Education
Associate degree in Paramedicine or equivalent education required for Oregon Paramedic licensure
Bachelor's degree in health, human services, or related field preferred
Experience
Minimum of two (2) years as a Paramedic on a 911 transporting ambulance or equivalent community health experience
Behavioral or mental health experience preferred
Licenses & Certifications
Valid Oregon Paramedic License and National Registry Paramedic certification
Valid Oregon Driver's License
BLS, ACLS, and PALS or PEPP certifications required
Community Health Worker (CHW) certification required within 12 months of hire
Community Paramedic (CP-C) certification preferred
Key Responsibilities
Provide high-quality patient care in accordance with clinical guidelines, protocols, and standards of care
Conduct in-home assessments, proactive and post-discharge follow-up visits, and specialty MIH visits
Respond as part of the Mobile Crisis Response team for behavioral health and crisis-related calls
Perform clinical interventions including point-of-care testing, EKG interpretation, wound care, medication reviews, and home safety assessments
Deliver patient and family education related to chronic disease management (e.g., diabetes, CHF, COPD, hypertension, fall risk)
Coordinate care with physicians, hospitals, allied health providers, and community resources
Provide both Basic and Advanced Life Support within scope of practice
Serve as ALS backup on emergent and non-emergent 911 ambulance calls as needed
Maintain accurate, timely electronic documentation and patient care reports in compliance with Oregon law
Utilize telehealth and technology platforms for physician consultation and longitudinal patient monitoring

People receiving this service have extra support specific to their individualized needs, and you will play a big role in ensuring they feel safe, happy, and connected. You will help them build relationships, try new things, and reach their goals. Your days might include shopping and cooking together, going out for new and/or fun activities, spending time with family and friends, helping with daily routines like personal care, ensuring housework is completed, and providing mentoring as the person plans their days ahead. You will also be part of a strong team, working together to create the best support possible by receiving additional training, being an active member of planning support, and playing a pivotal role in team development. You will provide on-call support for one week every five to seven weeks (Wednesday to Wednesday). This position requires you to have a valid driver's license and be eligible to drive agency vehicles.

Job Duties:

Participate in an on-call rotation for their assigned area. On a regular rotation, this position is on-call for their assigned area for 7 consecutive days in each rotation.
Physical Requirements:
Must be able to lift and transfer 25 pounds minimum; some positions may require more.
Appropriately and efficiently responding to emergencies and/or crises (including but not limited to the use of physical intervention).
Certifications and Training:
Must pass required DHS background check and Protective Service Investigations.
Most positions require a valid driver's license; some positions may be available for non-drivers.
Complete and maintain all certifications as required by the position, PCL, and other governing agencies.
Must be able to obtain and maintain certification to a satisfactory level in Oregon Intervention System (training provided if hired).
Must be able to obtain and maintain certification in CPR (training provided if hired).
Complete all duties related to being a Mandatory Abuse Reporter.
New hire orientation and on-site training must be completed within 60 days of hire.
