Center for Human Development (CHD) is currently seeking Shared Living Companion Staff. CHD's Shared Living program provides an alternative residential setting to the 24-hour group home. This is a shared living staff position in the East Longmeadow area. The individual receives Day support services from M-F and some hours during weekends and as a relief for the shared living provider. We are currently looking for a calm, polite, responsible person to provide client supervision during awake overnights. Open Positions: Part-time Companion hours (there's some flexibility with the days covered) Monday-Friday from 3pm-6pm and Saturday-Sunday 12pm-4pm. Pay rate: $20.00/hr Required: High School Diploma. The Primary Care Nurse Practitioner (APRN) or Physician Assistant (PA) Resident functions as a member of a medical team and applies to the treatment of patient's the medical and related skills acquired through the training and/or experience required for this position. Manages prescribed medication, performs physical health examinations, conducts preventive screening and care, develops and presents clear clinical impressions and recommendations that are specific and attainable. Develops and documents clear clinical formulations and medical treatment plans. Prescribes therapeutic interventions appropriate to identified problems. Administers drugs and biologicals as required by the Clinic. Demonstrates a basic knowledge of physical health concerns and effective medical interventions. Triages clinical emergencies and implements safety and consultation protocols. Utilizes evidence based and best medical practice. Monitors patients' self-care management and self-care skills and provides prevention, wellness, and pertinent medical education. Provides requisitions for clinical studies, diagnostic tests, and laboratory studies. Evaluates and interprets results to identify and assess patient's clinical problems and health care needs. Demonstrates knowledge of internal and external levels of care, referral processes and resources. Seeks additional supervision or consultation as needed and follows through with clinical directives. Provides service in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program. Must be able to pass a respirator fit test at time of hire and on an annual basis. Providers will treat HIV, Hepatitis C, and provider gender affirming care within their primary care practice. Support will be provided by the medical director and other provided resources. Provide services on the mobile unit, as required.
