The APP provides services within the APP scope of practice including the management of symptoms or disease processes related to acute and chronic conditions, assisting with communication and coordination of care, timely disease-specific education, as well as developing treatment plans that align with the patient's goals throughout the disease process. The APP would provide recommendations for and/or orders including pharmaceutical interventions/medications, laboratory, and diagnostic services as needed, and will work closely with the facility staff as well as patient and caregivers to maintain close engagement for optimal management. Assessment of needs may include utilization of additional IDT resources including social work or chaplaincy as needed.
